I received an invoice for $20.00, a charge that indicates Ciox did not find any medical records.When a client tells us that they have seen any doctor, we believe them, especially when they present dates and/or provide a document from the physician's officefor any date of service. Upon receipt of the invoice, we called the physician's office who provided us with 58 PAGES of medical records. I spent an hour ofhold time trying to Ciox and spent time speaking with the physician's office to get these records. My time, as well as the physician personnel, is worth something and it is greater than the $20.00 invoice that you expect me to pay.I was advised that the "invoice could not be voided and must be paid". Advised I will not pay and per rep **** would continue to bill until paid. Representatives need to do their job.. Do representatives get paid regardless of how *** they are at their job performance? The charges are passed on to our clients and we refuse to burden them with charges that are bogus. This is a continual problem with ****. I am doing what your representatives will not or cannot do. I am forced to find them by other means, and yet you expect the payment for non-performance by your *****Business Response

I am writing to request help in locating my medical records within Datavants repositories. I contacted Datavant in 2024 requesting to see if my medical records were in their repositories from past medical releases. I was told by their privacy officer that I would have to contact my medical provider for the information. I have not been able to get a complete accounting of disclosures of my medical record releases that have occurred through business associates at *** in ********. *** does not seem to know how to do an accounting of disclosures and just keeps sending me a log from their old system that is different from the one I have been provided in the past. The ***** Health privacy office has stopped responding to my requests and I have been referred to the Health Information Manager at *** in ******* who has also not been able to provide me with an actual accounting of where my medical records have been released to in the past. I have also not been able to get my medical records from ***, despite knowing that they were released by *** in 2014. Please refer me to someone at Datavant who can search for my medical records in Datavant repositories and help me with an accounting of the record releases because DMC Does not appear to be capable of giving me either. I am rejecting this response because: It was never explained to me whether or not my medical information is even in Datavant's repositories. I am trying to find out where my medical information is siloed, so that I may determine how it was breached. *** does not know where they have shared any of my information beyond the last 6 six years allowable through an accounting of disclosures. Please refer to someone who can determine if I am even in Datavant's system. Thank you.

Datavant/Ciox follows all HIPAA guidelines and all statutes in each state pertaining to the release of Health Information. Some Health systems centralize their billing records for multiple medical facilities causing separate invoices for the records. The medical records are located at the medical facility but the billing records may be located at a separate location. Thank youCustomer Answer

I am rejecting this response because: they use a portal for attorneys to request client's bills and records. You are given the option to choose multiple things such as records, bills, radiology, etc. on one request but then are billed separately. That is misleading. They need to overhaul their portal if is the case. If you are going to have to get them from separate facilities, then they need to clarify that.

A request for records was received and replied to on 1/24/25; we received correspondence a month later for the same charts. This is the SECOND time they cannot find the charts and last time one of them had the nerve to call my office and get rude with my staff. Now you have THE OWNER, and when you call, make sure you ask for ***** *******. We will be sending bills for re-work moving forward. If you do not know how to use a fax machine or be organized, call your boss, Highmark, and let them know because my office will not be doing re-work based on this company's inability to complete a task. I received an invoice for ****** for medical records when it should have been free because they were going to me. This was misbilled. My attorney requested it for me but the records were going to me. I've reviewed the complaint and found that the attorney submitted a request for records to be sent directly to them. The charge to fulfill the request was accurate per the Maryland statute ***. Code of Md. 4-304(c). The attorney requested the records and is therefore is responsible for invoice 485816496. The patient reached out to our customer service department concerning the invoice and was informed of this. Per Maryland statute the patient can request their records and receive them at no charge. I have been attempting to secure billing records on a PI case now for two months! I have sent in no less than seven requests for this information. Each time I have been told (1) no records available for patient (2) time period requested is not on the release (when it clearly is) and any number or responses that indicate I am not getting the records but I am getting billed each time they respond. To date here are the request numbers for this patient ********* (I personally called and spoke with a CIOX *** before this one was rejected for missing information and she confirmed it had everything needed to be processed correctly Yet IT WAS REJECTED AGAIN. *********; *********; *********; *********; *********; *********. I have requested these records YET AGAIN this morning and I do not feel my client should have to pay for CIOXs inability to clearly read what is being requested and instead randomly rejecting records requests in an attempt to run up their bills (which is ultimately what I believe they do) I really do not understand why any medical provider would use this incompetent group. It should not take 2 months to get copies of medical billing - I may have my clients register directly with the organizations and get their own billing records in an attempt to avoid using CIOX at this point as they are seriously delaying my ability to close my clients cases. I thought I had avoided all of this by calling and personally talking to a ***resentative at **** about the last request and she VERIFIED it was complete and should be fulfilled yet I received another reject inability to fulfill letter To date they have billed me SEVEN times for retrieving medical records and retrieved NOTHINGBusiness Response

I've reviewed the complaint and found the request had to be rejected several times due to the request documents were not acceptable to release the records. There were no charges for the rejection letters sent to the requester. As a company we held to a high standard by ***** regulation to provide copies of medical and billing records. Once the proper documentation was provided to us the records were processed and released in a timely manner. I apologize if the customer's experience with our services was a negative one. 6-21-16 Someone used my medical information and my SSN to come to the emergency room at ******************* ** in ********************. I was not there on that day and as a matter of fact i have never been to that emergency room, and my ******* only said i was there for pain management I spoke with billing, couldnt help me. I reported it to the *******************, have not heard back from them. I requested my medical records on 11/28/24 via **** mail, I received a letter yesterday on 12/19/24 almost 20 something days after mailing them. Saying they need more information to identify me as the person really requesting the information Why? They didnt protect my health information in the first place if someone was able to successfully use it in getting their own treatment with it while pretending to be me 12-20-24: I am going to file a complaint with this company who is in charge of my medical records doing business as CIOX health out of **********, so hopefully I will get somewhere with them because this is ridiculous they want my photo id in the request WHAT, my SSN AND DOB doesnt verify who I am but you let someone ***** nilly use my information 8 years ago? I have reviewed the complaint and find that the records were located and mailed to Mr. **** on 1/15/25 under my reference number 491078939. Datavant/Ciox partners with this hospital to process requests for copies of medical and billing records only. We are not involved in admissions to the hospital or generating the original records, medical or billing. ************* personal information erroneously was used at the hospital my suggestion would be to contact either their admissions office or their billing/business office to make any corrections that need to be made. In June of 2024, a request for medical records was forwarded to ***********************. In August, the invoice was paid by the firm. To date, this office has yet to receive the medical records. I, as the handling paralegal, have contacted **** and all their affiliated companies at least 7 times about this issue (and the other 3 accounts I have that have not been fulfilled). No one, to date, has helped me. I get one email after the other redirecting me to yet another email. When I call, i am placed on hold. As a matter of fact, I am currently on hold with them as I am writing this. We are now at 27 minutes and counting!!!!!I have several other accounts like this where we paid for medical records and did not receive them because of some internal issue they had with their portal. I have contacted their IT people for the portal and that has not worked either. If these issues are not resolved by the end of this year, I will be contacting the US Office of the Inspector General as this company has a duty to deliver these medical records in accordance with the rules and regulations under ***** and they are not doing so.I finally got through to someone after 34 minutes on hold, only for them to tell me that I needed to send another request to the provider so it can be issued to Ciox because WE AS A FIRM selected to have the documentation available for only 30 days. However, there are several different people in this firm requesting medical records on several different types of cases and they only send notification of information being available to download to one person. If that person does not handle that case, why would they download the records? PLUS, where is the money we already paid for these records? If I send another request, it will generate another invoice. Our client pays that in the long run and we will not charge them twice. SOMETHING NEEDS TO BE DONE ABOUT THIS!Business Response

I work for an attorney and we have been requesting records and bills from **** for many years. With the new system they have got in place it has now become much quicker, efficient, and easier to request our records and bills. I am not even sure if filing a complaint here is even the correct course to take, but I have no idea who to contact to get my issue resolved. For the past few months I have had issues paying for my records via my smartrequest account. I have tried multiple times now and it will not allow me to do anything. I had reached out via the chat option and was told everything looked fine and I should be able to pay for my records. I indicated I was not able to and the response was to call and pay over the phone. I have a lot of invoices and that seems like a total waste of mine and the other persons time. So I started using the paycioxhealth portal just to be able to access my records. that payment option is now down and I have no way to pay without calling in. All I would like is for this issue to be fixed on my account so I can pay like i m suppose to or at least give me contact to someone who can look into this issue for me, because its becoming a huge issue for me.Business Response
