    Main Street DME filed a fraudulent claim against my **************** account. It was filed 1/18/24 for supplies for bladder leakage in the amount of $1,960. My Mother has had no contact with this company or bladder issues.

    Provider Main Street DME, Inc. filed a services (supplies) with ******** for equipment we never requested and was paid almost $3000 for two claims. These supplies were never ordered or received. The doctor who was listed as ordering the equipment is *******************************, who we never saw. Looks like classic ******** fraud.

    My ******** number and secondary insurance were both charged for Dec. 2023, January 2024, and February 2024 for urinary catheters that I did not request, nor did I receive. This is a fraudulent use of my information. I tried to call Main Street DMI and was connected to a voice mail. I see in the newspaper that this is certainly a fraud and a scam.

    My elderly father was a victim of madicare fraud for medical supplies (urinary catheters) by this company. The charges showed up on his ******** statements and his supplemental was billed as well.Medicare and his supplemental company has been notified of the fraudulent charges and I've made several attempts to contact this company but you can't reach a person and calls go unanswered.This is a complete scam targeting ******** recipients.

    Claims submitted to ******** and insurance for services not received .     On two separate dates, 12/6/23 and 1/5/24, the company in question committed fraud by fraudulently charging my ******** account with $1,860 and $1960, supposedly for medical supplies that they sent me. I don't know who this company is, I never or anyone on my behalf requested anything from this company, never or anyone on my behalf received any medical supplies, and I don't know how they accessed my ******** number. Today I called ******** and filed two fraud reports with them and I am also reporting this as fraud to the ********, **, Police Commissioner. These people should stop defrauding people and should immediately return to ******** all the money they stole. When I spoke to ******** representative today they mentioned that this company has filed many fraudulent claims for reimbursement with them.

    for an entire year, this DME business has been trying to bill urinary catheter and drainage supplies to MY ******** and MY insurance when I HAVE NEVER USED THEIR BUSINESS OR HAD REASON TO> these are fraudulent ******** Claims.I have called ******** Fraud line and entered a complaint. I just received ANOTHER EOB that they are STILL TRYING TO COLLECT MONEY from ******** under my ******** number. THIS IS FRAUD

    According to a report from ********* this business claims it has sent me catheter supplies every month since March of this year. It has not, nor do I need one. ******** rejected all but the December claim of which it paid $1,461.38, passing $372.80 to UMR.
