      Good afternoon:To date a claim for ****** **** 9.7.21 has not been paid. We have sent over the op report several times , and *** keeps sending letters requesting the records again and ********* they are stating modifier is incorrect, which is not true. Please process claims for 9.7.21 for ******* and stop delaying the payment process for no good reason.

      I am fully disabled and have medical needs along with being disabled. I have been going to Gentle Dental for multiple teeth dying from my body enduring too many traumatic injuries. After I received the work I was sent a $1,847.98 bill. SSA Disability refuses to even pay me half of minimum wage so I would be homeless and/or dead if I was not living at my mother's place which is not safe for me but I can afford nowhere worth how little I am paid. I have no way to pay any of that bill and still have more dental work I need, another tooth dying, and can not return to the dentist I was using because of it. My neurology loses control of my autonomic nervous system when exposed to sound and this dentist was kind enough to make sure it was safely quiet for my appointments. This is not easy to find. I have no way to appeal or file a complaint to united healthcare because their site does not even let me log in and my Arizona long term care system case worker knows this and only ignores my disabilities and tells me to endanger my health and life by telling me to call phone numbers to do anything about it. My neurology can not handle sound so phone calls, especially with hold music are a painful death sentence for me. United Healthcare is going against the Americans with disabilities acts, give me no means to survive with my disabilities, and cares none that they are not taking care of my medical needs.

      In April 2025, I discovered someone fraudulently used my identity to enroll in a UnitedHealthcare (UHC) plan through the Health Insurance Marketplace. The policy ran from August 2024 to January 2025. I never applied for or authorized this coverage and only became aware of it when the *** requested a Form 1095-A, which I never received.The policy was submitted under my maiden name using old personal information. I have never used ********************** I was covered under my husbands Cigna plan, then had employer-sponsored *** coverage starting in 2024 through my own job. In January 2025, I called UHC during a medical emergency involving my childbut that call was regarding my employer-sponsored UHC plan, not the Marketplace plan. *** has incorrectly claimed that this call proves I was aware of the Marketplace ************************************* confirmed there are no applications under my married name, and UHC has not been able to explain how the plan was created. This policy was created without my consent and has caused tax delays, stress, and confusion. I have filed an identity theft report with the ***, submitted IRS Form *****, and have documentation of all employer-sponsored plans from 2023 through 2025.I am requesting:1.Immediate cancellation of the fraudulent Marketplace policy.2.Written confirmation that I am not liable for any coverage or tax credits.3.A full investigation into how my outdated information was used to create this policy.This situation must be rectified immediately.

      i canceled united health care and went devoted health in january 2025 . they continue to act like im with them . sending me bills ect.

      I went to the ** on 04/12/2024. ********************** only pain a small pittance of the bill. When asked why they did not pay for the ** visit, I was told, that it was only for accidents. This is not was I was told by Insurance salesman when I purchased it. As of now, *************************** is coming after for $26,000.00. I was lied and scammed. I want them to pay my medical bill. I have reached out to an attorney and am awaiting further direction.

      I submitted a dental reimbursement on 2/20/25 for $699. I have called *********** Insurance about my claim numerous times about the status of my claim, only to be told it is in the system and to be patient. My claim # is ***********. Three months have passed, and I still have not received reimbursement. I did notice many other members had similar complaints on the **** Hopefully BBB can be of assistance in this matter.

      United erroneously changed my coordination of benefits to ******** Part B although I am covered under my spouse's work plan. His company corrected the census which, by law, would make United the primary insurance even if I was enrolled in ******** Part B.The insurance broker has called United, I have called United, and my husband has called ******. Even with all of the supporting documentation from my husband's employer, they STILL have not updated my COB to make United the primary insurance. I now have over $2000 in unpaid claims for preventative services due to their erroneous processing. I cannot get ANYONE to address this issue.

      I have not been able to login to my online account since March. I have been on the phone with **************** 20+ times for an hour at least each time. I have gotten late notices in my email that i cannot resolve because I am locked out of my account. I keep getting transferred and no one can/will help me. This is about united healthcare in general not a specific location, but their customer service is horrible. this should be illegal.

      I am submitting claims for patient's that have United Healthcare ******** Advantage plans/ Optum. I am mailing the 1500 claim forms to the ** Box on the patient's ID cards. On over a dozen claims, when I call to check on claim status (after not receiving a payment), I am told that they never received the claim. I confirm ** Box number and am told that's correct. I've mailed with tracking numbers and this hasn't helped. Some claims I've been re-sending for close to a year and the answer is always.... sorry "no claim is showing". Getting a supervisor hasn't helped and being told they'll call back never happens.
