I ordered free in home Covid tests from Fast Lab thinking that I was on a free government site. I gave them my information & ordered the tests. When the order confirmation came through they had shipped two kinds of tests. One was the in home antigen kit that I expected but the other was what they called a *** test done at their lab & covered by ********* I did not order the *** test. Their confirmation said that *** tests could not be returned unused & if they were unused I would be billed directly. When the tests arrived there were no instructions. Materials were scarce & not clearly marked. It would be impossible to use them. I did not order the *** test. Upon thoroughly checking on our system, there was an order placed on October 31, 2023 under your name ********************* with free shipping. Our free shipping usually take upto 2-5 business days depending on your location. Your order was approved by our Telehealth/medical team on the same day because of the following reasons: Fever, Chills, Cough, Fatigue, Headache, Loss of taste/smell, and Sore throat.
In addtion, in our order form we have a default quantity of test. It can be can be found at the bottom of the order form when you placed your order and it can be edited, there might be a chance that when you processed your order you didn't edit the quatity of your test kits. Our Antigen/home test kit requires to come with our *** test. The Fast Lab COVID-19 at-home *** kit is a molecular test, also known as a reverse polymerase chain reaction (RT-***) test. Our *** test is the most accurate way of detecting if you have COVID-19. The *** test is required to be returned to us within 7 days. We do want to be transparent with you; kits that are mailed back to us unused will be billed directly to you as they become unusable.
To avoid being billed, we kindly request that you simply use the kit and ship it back to us at no cost. You may keep the antigen test kits for future use as long as they are not expired, and you will see the details on the box. You can always reach us for a fast result, and we will be happy to assist you.
We were able to shipped 2 *** and 7 Antigen test kits makes it 9 test kits in total, the kits are shipped in separate packages with a delivery date interval. This arrangement of shipment is to give you the chance to cancel the remaining scheduled if you don't need the rest of your ordered test kits anymore. Please know that we can only cancel orders that are not yet processed or orders that are not yet scheduled to ship.
To be clear, we were able to process the first shipment on October 31, 2023. The last shipment was processed on November 13, 2023. Since all of the test kits have been shipped, we will no longer contact you. We will no longer send any test kits. We will no longer bill your insurance for any kits from our company, as we will no longer send you any.
With regards to the *** instruction inquiry, we checked our records, and we found out that on November 6, 2023, you reached out to us asking about the *** instruction. One of our customer support representatives replied to your chat on our helpdesk site, mentioning the link where you can find the instructions for taking the *** test.
Furthermore, please be guided that our COVID-19 test kits are free of charge provided that you have a valid and eligible insurance, you wont be paying out of your pocket. Once verified eligible, you will receive your test kits and your insurance will cover for you and your insurance will pay Fast Lab. If verified ineligible no test kits will be sent to you.
Rest assure that we will continue to work with your insurance company with regards of this claim and as long as your insurance is active and up to date there will be no charge to you. Your insurance company will pay Fast Lab and we will not be billing you for any balance. Please take note as well that we are unable to do billing adjustment due to the reason that all of the test kits were already sent to you.

I no longer have the original date, but sometime in late September, I believe, I sent a request to Fast Lab Technologies LLC for what they claimed was a free covid test. I received a box a week or so later, probably in early October. I never used or returned the test. Subsequently, they started sending me other packages that were never requested. To date I have received four more packages; three flat mailers and one more box. They are not labelled as to their contents. They were shipped on *****, *****, *****, and *****. I checked with my insurance company and so far they have not received any bills. Again, I did not request any of the last four packages and do not want any more to be sent. On September 24th 2023, I ordered covid test from Fast Lab Technologies LLC as they advertised on their web site that it's free covid test. After that I did not order again and they keep sending me the covid test packages weekly and I sent back to the sender every time I received it. I called their number ************** but it referred me to use the chat on their web site fastlabtech.com and they did not take my request to stop sending me the covid test packages. They sent medical claims to my insurance company. I called my insurance to inform them about this company and request them to stop accepting the claims from fast lab. We thoroughly checked your order information in our system and we received your order for 2 PCR and 8 Antigen test kits dated September 25, 2023 under your name Bee ********* with free shipping. For free shipping, typically it takes 2-5 business days to deliver your first package. The test kit packages are delivered with date interval until all total number of test kits is shipped.
This delivery arrangement was intended for all the customer/patient including you to have the chance to cancel the scheduled order if you no longer need it. Upon checking, your first package was processed 09/25/2023 while the last package was processed 11/27/2023, and all of your orders were successfully delivered to the address you provided with us. Bill sent to my Secondary insurance on 10-01-2023, second bill sent to provider 10-02-2023, trying to get me to pay them $650, this bill came attached to my Doctor bill for surgery I had don't know how they got info and were able to do that??? Just want my billing dropped, also Mayo health system made a where of this problem, so the can watch, that must be where they got info from for my secondary, also probably also got my medicare # but not sure, so far they have not tried billing medicare.

For free shipping, typically it takes 2-5 business days to deliver your first package. The test kit packages are delivered with date interval until all total number of test kits is shipped.
This delivery arrangement was intended for all the customer/patient including you to have the chance to cancel the scheduled order if you no longer need it. Upon checking, your first package was processed 09/25/2023 while the last package was processed 10/24/2023, and all of your orders were successfully delivered to the address you provided with us.
