    I received information from you attempting to collect the above-listed alleged debt. Your initial communication to me of the alleged-debt was within 30 days ago. Please be further advised, I dispute the validity of the alleged debt in its entirety. Further I request verification of the alleged debt, including, the name and address of the original creditor, along with a copy of a signed contract with the creditor. Where’s my HIPPA release form? I am aware that you have directly violated the regulations of the HIPAA Act of 1996. I never signed a HIPAA form to authorize your company to have access to my medical records or procedures that I went through. Additionally any attempts to harm my credit history and rating by updating or changing dates after you have been informed that the debt is expired, is a direct violation of the FDCPA. Any abuse to my credit rating on your part will be met with all recourse available to me by the law. You are illegally attempting to collect an alleged debt and money that I do not owe. As per the law, cease and desist all collection activity, including credit reporting. Promptly request deletion of any prior credit reporting to credit reporting agencies.

    Please be advised, Commonwealth Financial Systems, Inc. is a  third-party collection agency and as such, we do not have contracts directly with consumers. We service accounts on behalf of our clients with whom we share a contract and who has purchased the account from the original creditor. The original creditor has a right to sell/charge-off accounts after they become seriously past due. While the consumer did not create the debt with the collection agency, the original creditor has sold/charged off the original debt to our client who then assigned it to our office for collection. This is what gives the collection agency the ability to collect for an outstanding debt. 

    As for HIPAA authorization, the HIPAA law permits creditors to share information with its Business Associates necessary to collect on medical debt. The HIPAA rules require that covered entities and business associates enter into contracts to ensure protected health information is appropriately safeguarded. Commonwealth Financial Systems, Inc. operates under a Business Associates Agreement with our client which certifies that we agree to not share private health information with anyone other than the consumer, or authorized party, and we further agree to keep records secure. Our company has been fully compliant with HIPAA requirements concerning privacy of medical records. 

    The debt, which originated from physicians' services provided on 02/12/2016. In regards to *** ******'s reference the debt is expired, the FCRA compliance date for credit reporting is 7 years from the date of service or date of first delinquency. The service date, used for FCRA purposes, is 02/12/2016, making the account eligible within the credit reporting time limit. If in relation to the statute of limitations, the statute applies to the maximum time which legal proceedings may be initiated; Commonwealth Financial Systems, Inc. does not bring legal action against consumers.

    The record shows receipt of a message sent by *** ******,  via our company website,  on 11/11/2022. *** ****** disputed the validity of the alleged debt. Our office began our reasonable investigation of the disputed information and reached out to our client for validation of the debt. Three days later  *** ****** is requesting validation while at the same time telling us to cease and desist all collection activity in the said Complaint. in response to consumer's  request, we have closed the account and will return it to our client.  *** ****** will have no further contact by our office regarding this account  A deletion request has been sent to the 3 major credit reporting agencies for the tradeline(s) to be removed from credit reporting.

    Commonwealth Financial Systems, Inc. is a  third-party collection agency and as such, we do not have contracts directly with consumers. We service accounts on behalf of our client with whom we share a contract and who has purchased the account from the original creditor, ***** ****** *********.  The original creditor has a right to sell/charge-off accounts after they become seriously past due. While the consumer did not create the debt with Commonwealth Financial Systems Inc., the original creditor has sold/charged off the original debt to our client who then assigned it to our office for collection. This is what gives our collection agency the ability to collect for an outstanding debt. 
    The subject account is the result of physicians' services provided to minor child, ****, on November 15, 2017 at ***** ****** ************ Hospital Dallas. Following insurance payments/adj, there remained a balance on the account which has been placed with our office as unresolved.
    Please be  advised Commonwealth Financial Systems, Inc. completed an investigation into the validity of the debt in response to a dispute submitted to the credit bureau(s) in July 2022. The investigation confirmed the accuracy of the account data and the balance due was verified.  A copy of the detailed billing statement from the original creditor was mailed to *** ****** on 7/28/2022 for her records. 
    Our office has no record of receipt of Complaint ******-******* and no results were found for complainant, nor Complaint ID# ******-*******,  on the referenced regulatory agency's complaint platform.
    Though no violation was found, and consumer did not provide any documentation showing the debt is not owed, we will  honor the request for no further contact by our office regarding the above referenced account. We closed the account and will return it to our client advising them Commonwealth Financial Systems, Inc. will no longer be servicing the account.   We have submitted for tradeline deletion to the three major credit reporting agencies - ********, ******* and **********.

    I recently received notification of a collection item on my credit reports reported by Commonwealth Financial Systems that based on the original creditor was a service provided when I was a minor that I am not legally financially responsible for. The original creditor is ***** ********* ********** and there was no date on the report but my mom called and the **************** advised it was from January 2016, by the physician that treated my ankle injury. At the time my mom signed for and was the financially responsible parent. I have disputed this 3 times on ******** and they will not remove, Per the hospital, it is illegal to list this on my credit as I was minor. Not to mention, there has been NO communication from this Commonwealth to provide any type of documentation of outstanding debt that remains.

    Please note, the date of birth of record is 12/28/1998. The debt in question was the result of emergency physicians' services provided  on January 12, 2017 at ************** HOSPITAL.   These services were provided by a member of ***** ********* ********** *** medical group, the original creditor of the account. There was a remaining balance on the account after payments/adjustments, which ***** Emergency Physicians later sold/charged off  to our client after it had become delinquent. Our client, the current creditor of the account, assigned it to our office on 08/12/2022 for collections. Prior to credit reporting, our office sent written notification of the debt to **. *********;on 09/01/2022.  In accordance with Regulation F,  the account was then monitored for a no less than 14 days to allow for notice of a deliverability issue with the address. No notice indicating a deliverability issue was received. 

    On 09/22/2022, our office received a notice from the credit bureau(s) that the consumer was disputing the debt. We reviewed all relevant information regarding the dispute that the CRA received from the consumer and conducted a reasonable investigation of the disputed information. The investigation confirmed the accuracy of the information being reported to the credit bureaus and and that we were attempting to collect the debt from the right person. We obtained verification of the debt and forwarded a copy of the supporting document  to ************. The document was sent to the address provided by consumer with his dispute and was not returned as undeliverable.  ************ can contact our office at ************** should another copy be needed. 

    Thank you for your patience while we completed our investigation of ************** claims.  We will continue to note the account as disputed.

    Commonwealth Financial Systems, Inc. is a  third-party collection agency and as such, we do not have contracts directly with consumers. We service accounts on behalf of our clients with whom we share a contract and who has purchased the account from the original creditor. The original creditor has a right to sell/charge-off accounts after they become seriously past due. Though an account is charged off or sold, this does not satisfy the debt, nor does it affect the consumer's responsibility for the amount owed.   While the consumer did not create the debt with the collection agency, our client, who acquired all ownership rights upon purchase, including the rights to collect on the account, has assigned the account to our office for collection. This is what gives the collection agency the ability to collect for an outstanding debt.   

    On June 30, 2022 CFSI received a notice from the credit bureaus the consumer was disputing the debt stating account is inaccurate.  In response to the dispute, we conducted a reasonable investigation of the disputed information and confirmed the  information in our records.  We obtained validation of the debt in the process. Our office forwarded a copy of the supporting document to **************** on July 12, 2022. 

    Commonwealth Financial Systems, Inc has provided **************** with both a validation of debt notice and the itemized billing statement. We have provided consumer all necessary information required by the Fair Debt Collection Practices Act (FDCPA) and applicable State law. Commonwealth Financial Systems, Inc. believes it has met its standard for verification under Federal law and the investigation is complete on our end.  

    Please note, this account is no longer being serviced by our office. The account was closed October 03, 2022 and returned to our client. A deletion request was submitted to the three major credit reporting agencies - ********, ******* and **********, to delete all tradelines associated with the account from credit reporting.

    credit score dropped after false collection from company i haven't been to any medical facilities in the past year or better and all of a sudden these guys put a collection on me for some medical thing that never happened

    Please be advised, the account in question is the result of emergency physicians' services provided on 11/07/2016 by the ****************************** medical group. ******************************, the original creditor of the account, later sold/charged off the debt to our client, the current creditor, who then assigned it to our office on 09/12/2022 for collection.

     Prior to credit reporting, our office sent  *********** notification of the debt. The notice was sent 10/06/2022  in an electronic communication. In compliance with Regulation F, after sending the notification we monitored the account for a minimum of 14 days to allow for notice of deliverability issues with the email. After 14 days, when no notice was  received, the account was sent to be credit reported. 

    On 10/24/2022 our office received notice from the credit bureaus the consumer was disputing the debt stating he was not aware of the debt in collections. We obtained the validation document containing detailed  information about the debt and forwarded a copy  to consumer on 11/10/2022 via USPS postal mail.

(‘CFSI’) take our consumer protection obligations very seriously and have dedicated significant resources to our compliance-related functions. The above referenced account was reviewed fully in conjunction with the consumer’s complaint, and I believe the following will address the issues and concerns set forth in the complaint. 

    Commonwealth Financial Systems, Inc. is a third-party collection agency that collects on behalf of our clients.  The account in question is a medical debt that originated with ********* ********** of ******* *******, who is the original creditor.  Our client purchased the original debt from ********* ********** of ******* *******,  acquiring all ownership rights, including the rights to collect on the account. While the consumer did not create the debt with our agency, our client, the current creditor of the account, assigned it to our office for collection. This is what gives our agency the ability to collect for an outstanding debt. 

    The  account in question was placed with our office for collections on 01/24/2022.   In accordance with 12 CFR 1006 of Fair Debt Collection Practices Act (Regulation F), our office sent *** *** ****** written notification of the debt formatted in accordance with the Consumer Financial  Protection Bureau’s Model Validation Notice. The notice was sent January 31, 2022  to the address reflecting in said Complaint.  The account was then monitored  for a minimum of 14 days to allow for notice of a deliverability issue with the address. After 14 days, when no undeliverability notice was received, the account was sent to be credit reported.

    On 04/26/2022, our office received written correspondence from *** ****** in which he disputed the debt and requested validation of the debt. We conducted a reasonable investigation  and confirmed the accuracy of the information being credit reported. We obtained validation of the debt, as requested by *** ******,  which was forwarded to him on 05/24/2022.   *** ****** can a copy  at 1-800-848-2170, if needed.

    Please note, licensing information is available to the public and can be found online within the consumer's state.

    A collection for $637 showed up on my report May 2022. I haven’t lived in NM since 2014. I have not been to the emergency room out of my current state. There’s no doctor listed, what the original bill was for, the reason the VA didn’t pay the bill or if they even attempted to get it paid using my VA insurance, there’s no date of visit, no info at all. Just shows collections for an amount I have no clue what is for. The last time I did use ***** ****** ******* I had a sinus infection and paid the bill cash immediately cuz I didn’t have insurance. All other medical needs have been met by the VA since 2013. I have disputed this twice on my credit report, but they keep coming back.

    We thank you for the opportunity to respond to Complaint ********  submitted by your office on behalf of  *****************************. We at Commonwealth Financial Systems Inc. (‘CFSI’) take our consumer protection obligations very seriously and have dedicated significant resources to our compliance-related functions. The above referenced account is the result of emergency physician services provided at ****************************** on 09/29/2016 by the ***** ********* *****. Please note, the physician bills separate from the hospital. The debt was later purchased by our client, who assigned it to our office for collections.

    Prior to credit reporting,  written notification of the debt was sent to **************** on June 01, 2022.   In accordance with Regulation F, after sending the notification, we then monitored the account for a minimum of 14 days to allow for notice of deliverability issues with that address. The notice was not returned as undeliverable, nor was a notice received indicating a deliverability issue.  

    On July 08, 2022, our office received a notice from the credit bureaus the consumer was disputing the debt. In response to the dispute, Commonwealth Financial Systems Inc. conducted a reasonable investigation, obtaining  verification of the debt in the process,  and confirmed the accuracy of the  information being credit reported. The supporting document validating the debt was forwarded to consumer on August 03, 2022, sent to the Alabama consumer provided in her dispute to the credit burearu(s). 

    Please be advised, the account in question has closed since the submission of Complaint ********,however, we would be more than happy to assist consumer in resolving the debt. Should **************** have documentation or information showing the debt is not owed, it can be mailed to our office at:Commonwealth Financial Systems, Inc., 245 Main Street, Dickson City, PA 18519,- or emailed to:  [email protected], and we  will forward to our client for investigation.     In response to the complaint sent on behalf of ***************************, please be advised we received a complaint in August 2022 submitted on *******************'s behalf  from another regulatory agency with the same dispute. Please refer to consumer's letter,  dated August 01, 2022, that was attached in said complaint. This letter accompanied the previous complaint in August. The matter was investigated and we verified the accuracy of the debt. The supporting document was sent to ****************** for review. 

    Please note, this matter has been investigated and the dispute was resolved.  The referenced account was closed on 9/16/2022 and Commonwealth Financial Systems Inc. is no longer servicing the account. Our records indicate that on 09/16/2022 a deletion request was sent to the 3 major credit reporting agencies to remove the account from credit reporting.
