      I have had billing issues with Perfect Teeth 4 Hills office in *********** over the past year. Prior to that, we were satisfied customers with their services and ******** for approximately 18 years. Then personnel changed. The past year or so they have consistently failed to follow their contractual obligations with our insurance company, Delta Dental (DD). They charge more than the approved DD rates, they charge for procedures not approved by DD (which are considered included with other approved procedures) and fail to properly adjust our account to reflect DD approved rates. I have attempted numerous times to resolve these problems with the office manager, office owner, billing department and headquarters. I get no response. Calls to the corporate headquarters phone numbers listed on their website are constantly busy day and night (i.e. bogus numbers). A letter to their corporate headquarters address on their website was returned Forward Address Expired. They have no mechanism to question and resolve billing questions. Finally, out of frustration, we filed a complaint to DD and transferred to another company for dental services.
