Perfect Teeth stated that 6 teeth needed to be pulled out so I agreed to have one implant done by their oral surgeon. Well, they tried to pull a tooth that had nothing wrong with it. This was confirmed by another Dentist. They put the Implant in the wrong place. So now I still have a gapping hole in my smile. Perfect teeth also said I needed a Nasal lift, now I am really questioning if this really was necessary $6000 later for everything. Needless to say, I was sedated during this procedure so I did not know the implant was being put in the wrong place. They tell you you need things you don't, they do things wrong. I tried to reason with them and their response was "I will have someone call you concerning this". Well, they don't call you back even after several attempts to reach someone other than the front desk person. I talked to a Lawyer and the Lawyer told me that the insurance these dentists have will just drain you dry and counter *** you for attorney fees. I just wanted the money I spent $1375 on the implant returned so I could have the implant put in the correct place. I just want to clarify this all happened at one of the Perfect teeth in **************** ********. **-20-19 Covid Happened. Just found out on 10-5-22 the implant is in the wrong spot. supposed to be in #** but it is in #**.This complaint is for ***************************Initial Complaint

At a regular dental hygiene visit on 8/18/2022, ************** recommended that it was time for a crown on one of my molars. The front desk printed out an estimate for me that totaled $895.50 which states "The above fees are valid for 90 days from the date above without re-evaluation of conditions." The hardcopy says nothing about the necessity to sign it & have the front desk make a copy of it for their records. Also, my family uses the insurance plan that Perfect Teeth offers.Upon arriving for the 1st appointment for my crown on 11/07/2022, the front desk gave me a treatment plan to sign that said the total was $1,383. I commented to the front desk person that this was a higher amount than I had been quoted. Since I didn't bring the quote with me, I asked if she had a record of it on file. She said she did not. I went ahead with the appointment, not wanting to jerk ************** around by cancelling last minute.I returned that afternoon with the hardcopy quote I had been given in August. The front desk person said she was a temp & that a new office manager was starting in 2 days. She copied the quote & put a sticky note on it.I returned 3 days later on 11/10/2022. I spoke with someone that appeared to be training the new office manager & eventually with another person that was her boss. They sympathized with me, but said that since I hadn't signed the original quote & left a copy with the office, they could not refund the full $487.50. While the uninsured amount of the bill hadn't changed, they said the amount the insurance covers had changed & that's why there's a difference. Never in the 7 years of being a patient there have I been told that I should sign a quote & make sure they make a copy of it. They offered a $200 refund, & ironically could not give me any hardcopy proof that they had done so. I'm supposed to trust that it's there on my account. Dental appointment date: 04/04/22. Requested a 'Checkup' with a named dentist at Perfect Teeth (***************). Date of appointment, I presented my dental insurance card (which they scanned) and took me in the back for an x-ray. First Bitewing did not turn out so technician came back, apologized, and repeated the same x-ray of the same tooth. When a different dentist came in, they showed me the one x-ray and said there was no decay showing on that tooth. Discussed my options, and received a referral to an Oral Surgeon. At the time, I did not have any other x-ray. Paid my co-pay of $60 and left with a paper showing my charges for that day. I didn't know what the x-********* had was called. For months, I received a statement showing that I owed them $154.00. Kept trying to call them, but was treated rudely by phone, and never did speak with anyone from billing. Finally in July, I spoke to dental insurance supervisor who said no claims had been filed for my dental visit on April 8th. She said that if I paid my co-pay of $60 that should be all I owed Perfect Teeth. Each time I received a Statement, I tried to contact someone at the billing office. I told them when I called in May that I did not have two x-rays, only the bitewing was done. Other times, I was not able to reach a human by phone. In early September, I received a notice from a DEBT COLLECTOR, trying to collect $154 for Perfect Teeth. I called them right away, saying I wanted to dispute the charges. I mailed them a packet of each of the statements I had received by that time, along with all my notes of my attempts to resolve the matter, along with notes I made whenever I was able to speak with my dental insurance representative. I made a copy of that same packet and sent it to Perfect Teeth at the same time. No reply saying the matter had been resolved has been received. My claim was paid on Sep 18th, but now I supposedly owe them $30. I DO NOT OWE them anything.Initial Complaint

Hello, I have been trying to reach perfect teeth for the past few months about a bill I received but have not heard back from anyone. This bill has already gone to collections but I have been reaching out to get an explanation of this bill as one was not provided in any of the statements I have received. I initially left a message at their website around July for an explanation of this bill but heard nothing back. This option is no longer available on their website. I tried calling the number listed in the statement and collection notice, and it directed me to contact *********************************** It has been 45 days since I first reached out via email and have not heard back. I wrote on September 15th and October 9th. For context: The first statement I received was for 12/15/2021. This was over a year since the last time I made a visit to the clinic, and a visit that required out-of-pocket payment from me so I am confused as to why I am receiving this now. I had not visited Perfect Teeth since the end of 2019. Based on my last payment, the insurance should have covered the rest. ON April 29,2021 I paid in full with Aetna Discount Plan $360 for a dental procedure. But Perfect Teeth for some reason submitted another claim to Delta and got rejected even I already paid in full and told them it's not covered by Delta. Then on August 9th this year, my husband went for a cleaning. Perfect Teeth insisted that he paid $252.50 for the rejected claim. Later I called the office and explained the mistake to the manager and she agreed to handle it and call me back, but she never called. Now the office is closed and we tried many phone numbers and getting no where to get the money back . We need someone from Perfect Teeth to contact us directly to resolve this issue. This location is a nightmare. Prior to my first appointment my previous dental office emailed 3x my records and cc:d me which I got. 1st appointment, they couldn't get my records as they stated their email was down. They took x-rays and the Dentist never touched my teeth nor cleaned them- said I need to go to a Periodontist. The Dental Assistant had me wait in the dental chair while she went to get the referral. After several minutes, she ended up forgetting about me and walked by me while eating her lunch. When we walked to the front desk she told the office Manager to get me the referral. The Office Manager must not have heard her because she asked me what I wanted. I had to wait several extra minutes for her to argue with the other girl at the desk about where her pen went. Afterwards, she told me she couldn't give me a copy of the referral because their printers were down also. I had to take a picture of it with my cell phone. Your Periodontist never cleaned my teeth but instead told me for $6,000 he can pull 6 of my teeth instead. What?!!! Please know that it took half a year and two referrals before I was able to see a Periodontist as your **** location canceled my appointment without notification, the second time it was because they said the Periodontist was fired. We are now in September and still no cleaning. My son had an appointment at your **** location to see about getting his wisdom teeth extracted, when the Dentist told me that they will run multiple IV's in his arms with 5 different narcotics that will knock him out "but don't worry, we will have a handful of nurses standing by" I ran out of that place. My husband was seen in July and September, paid $1200 for impressions to get dentures. After sometime and no response, we called and were informed they had only the top and waiting on bottoms. ****** confirmed today they can't find his impressions and will have to start all over. DO NOT Use perfect teeth on Union and Academy. I've had my braces for almost a year. Reoccurring issues the entire time. First time they took $660 out of my account when that was not my payment plan. Didn't get that back for almost a month. ******** at the front deck knows nothing EVER. I've been waiting for a call back from the manager for a week now. Nothing! I was seen last month and apparently I had a balance due but they did not say anything to me about it when I was seen. A week ago I got a letter in the mail from a collection agency saying I owe them money.Perfect teeth sent my bill to ***************** still a patient. They have now messed up my credit. I have reviewed your account with Perfect Teeth Union & Academy - Looking at this account you have been delinquent on your monthly Orthodontic autopays due to your credit card declining.
You also had an incident in our office 8/30/2022 - Patient presented to the office at 4pm with a 4:30 appt. During check-in, she informed me that she wanted to discuss her account balance. I responded sure, I can help you and proceeded to look into her account. I reviewed her ledger and said, you have an account balance of $799.95, looks like your auto draft payment declined. The patient responds you guys sent me to collections, you guys "F" up my credit and I am not going to pay you and them. I asked, would you like to make a payment today? She did not allow me to complete my sentence and cut me off and said "B" are you deaf. I then responded, i understand you are frustrated, however the disrespect and language is not acceptable. I can help you with your account, but I will not continue to be disrespected. She then jumped around the plastic barrier in my face and proceeded to yell more profane language. I walked away from her and walked around the office once. I circled back to her asked her to leave the office and informed her we will not be completing her appt today. The patient left the office after telling patients waiting do not have ortho done here they will f up your account.
You signed our ********************* Agreement on 9/7/2021 authorized Perfect Teeth to deduct $266.65 - Those payment declined multiple times and your account was sent to collections.

I have been sent multiple bills from Perfect Teeth on *******. in ******** ******** saying that I owed $67. I contacted the office in the summer of 2022 and talked to ******** who told me that I didnt owe anything and to ignore future billing. But I kept being billed and then I got letters saying that it was going to collection. So I called back in August 2022 and talked to ******** again and she said it was going to be sent to a higher level review,. I was told by her that there would be no further collection agency action until it was resolved. She told me that she would get back to me within a week and that never happened. Now I have a letter from a collection agency. I am very unhappy with this quagmire. If I owe money I will pay it but the last phone conversation was that Perfect Teeth was looking into it. Went ahead and re-submitted claim directly on DXC after checking eligibility which brought patient up no problem with the correct ID#.

I have had billing issues with Perfect Teeth for the past year. Prior to that, we were a family of 4 satisfied customers for years. Our previous dentist went out for surgery, never returned and then personnel changed. I have been overcharged due to issues on their end and things not being billed appropriately. I was assured by multiple staff members that the charges were being resubmitted and fixed. It has been over a year and nothing has been corrected. I am now sitting in collections with no resolution. Since then, the office has closed, the sister has closed, the corporate office has a fake#, the other corporate office leads you to nowhere. I am rejecting this response because: I have working with Perfect Teeth for 3 years now to dispute a charge for $575.40. While they answer their phone, take copious notes, say its being worked on...there has been no resolution and no contact from their end calling me back (although every time they say they will). There is no corporate number to call and I need this charge removed. The charge occurred 3 years ago due to a misbilling by a new employee. It was recognized immediately by the original office manager (*******) who worked to "correct" it. She quit so ****** worked on it. She moved so ******** worked on it. ****** is back, ******** has transferred me info along and I have no idea how to get this resolved. It is beyond frustrating and disrespectful. I would love your help in getting this resolved.

I went into perfect teeth for consultation. Was given a treatment plan and estimated cost of what my charges would be and what the insurance would pay. I was having all my upper teeth removed and getting immediate dentures. 1- I was not given immediate dentures like I was supposed to and now its too late because no impressions were made. Now I have to wait over 3 months!2- my care credit card was charged ****** dollars and my out of pocket expense was only ****** dollars. I have called and asked that my card be credited and dont know why they even charged that amount. They say it would take up to 60 days. NOT acceptable. It did not take 60 days for you to get paid!3- I just received a call from ******* at your ********************************************************************, ******* stated that they do not have a dentist there and are sending me to another dental office .Really?? How is there no dentist at their office? I was scheduled to go in on august 15 to hopefully make impressions for my upper denture. They started my treatment but now wont finish it! Unacceptable. I want my charges on my credit card refunded asap! The only charge should be for 184 dollars, NOT 870 dollars. My insurance has paid you their portion.
