*************** of Dental Assoc in ******** said I need 4 teeth removed and to have them replaced with a 6-unit bridge and adjacent to that a 3-unit bridge all on the lower part of my mouth. ON 10-10-2023 MY POLICY PAID THE ***** OF $423.00 to pull the bad teeth. Then on 11/20/23 I needed all my 4 crowns that hold the 2 bridges in place restored. At this time, I spoke with *************** of what needed to be corrected. As he prepared a plan for the Bridges etc. I explained to him I did not wish ALL PORCELAIN CROWNS and BRIDGES at an extra cost to me of $2571.00 Today 11/20/23 he was to Restore all the Old Crowns and take a full set of impressions for the lab to build up the two Bridges. This work was APPROVED FOR PAYMENT in the amount of $7845.00 by my insurance co. Network Health of *********** and Delta Dental ******** of Minnesota who Approved the Allowed Amount to be paid to *************** listed above. Yes, It's For all Crowns and Bridges to be covered with Porcelain fused to predominantly base Metal. WITH THIS MADE BY THE *** OF METAL, YES BEEING METAL I WOULD OWE NOTHING. Now if *************** made the Crowns and Bridges out of ALL Porcelain as he wanted to (In His Original Treatment Plan) then I would be charged an extra amount for the ALL PORCELIN ITEMS, THAT EXTRA AMOUNT IS $2571.00 for all porcelain! yes, *************** wanted me to accept his plan for all Porcelain just to make his added income jump up another 33%. to $2571.00 I'm 77 years old and live on SSI that will not support this extra amount as he proposed! I showed him a photo of the item I wanted also **************** was informed I Wanted him to remove *** Etchings on this same day, these were added to his overall plan, at a cost to me of $420.00 it was totally not needed, This was deleted YES REMOVED when his order went into the Lab. I requested ****** Delete all that I requested Yet it is now on my bill for $2571.00 I don't owe it. Ask him I we had the conversation to remove the artwork and the rest. I was seen in the office on 3/27/2024 for a root canal and crown. When Dental Associates called to remind me of the appointment, they told me there would be a balance due that day. When I checked in for the appointment they asked for payment. I used 2 cards for payment, 1 HSA and 1 credit card. When starting the root canal, it was decided that the tooth was too badly cracked and an extraction was recommended. They were able to fit me in the same day, and the assistant working with the doctor doing the root canal assured me that my account would be credited for the payment I already made because the extraction was considerably less money. I called the Dental Associates billing office on Friday 3/29/2024 and was told that my cards were not credited for the difference. At that time, the billing person said it would take 4 weeks to receive a refund check for the difference. I was transferred to the clinic and the individual on the phone told me she didn't know what I was asking. I gave up at that point because she did not offer to let me speak to someone who could understand, thinking it would be resolved. When the money was not credited to my HSA card, and the bill from the credit card came with the charge on it, I called the billing office again. I was instructed to leave a message, which I did, and my call was not returned. I called the office again on 4/8/24 and was told it would *********** weeks for the refund because it was in claims. They have failed to return 3 calls as of today's date, including a call in which I was hung up on. Basically they are holding a refund for a service they did not perform which I believe is fraud. The amount I paid was *******, the out of pocket cost of the extraction procedure was $90.60, so I am owed a refund of $1052.40. A claim for $239.40 was paid electronically to Dental Associates by my insurance company on 3/30/24. I believe it is unreasonable for my refund to be held for 6 weeks.Business Response

Went to have a tooth extracted in June 2023 to dental Associates Main St., *********. Ended up to useless dentist extracting a tooth that was not supposed to be polled. Left the root tips in and then fighting with that side for almost a year now they said they were going to replace it with an implant, but they cant get that job done right either keeps getting infected and they tell me I just have to wait. I have been a patient of Dental Associates-********* for many years however, I will no be a customer due to their unethical practices, mistakes, lack of training and scheduling issues. I had to make several appointments to try to get a cyst removed. I was told this could be cancerous so I made appointment to get it removed immediately. My appointment was pushed back multiple times due to them stating the ******* was not available. I finally made an appointment in January of 2024 just to have a conciliation. I was given a Treatment plan with the estimate cost of *******. The Dr. recommended an $80.00 product and told me it could help with the surgery procedure. I had to pay approximately $250.00 out of pocket plus the $80 for the product. I was told my out of pocket cost would be approximately $300.00. I schedule the appointment for surgery on March 4, 2024. When I checked in, they told me that my out of pocket cost was $3000.00 and my insurance was cancelled in February. I called my insurance provider who stated that was false and I was covered. When I went back to speak with the receptionist, she told me there were 2 additional procedures that were added on to my bill that I didnt know anything about. She couldnt explain it to me how or why these procedures were added without my knowledge. I told her my insurance was not terminated and I had a representative over the phone. They did speak to one another and all of a sudden, the receptionist stated I had insurance and they didnt know what was covered for the surgery. I did speak to a supervisor and an assistance. The supervisor didnt know what the procedure was, why I was being billed for additional procedures. They both stated Dental Associates employee added procedures on to my plan without my knowledge and said it was staffing/training issues. The supervisor said she would tell staff not to do it again. I also was told the removal of my cyst was never on the original bill. I told them that was the main reason why the surgery was happening. They could not explain to me how this happened besides it being a training issue with their staff. I was told someone should had contacted me regarding this issue. They did apologize and stated it was a mistake the company made. I was told the receptionist was in training however there was no one there training with her. I was then told my dental insurance would not cover anything but it was my medical procedure and my medical insurance would have to cover it. I asked why was this not asked back in January. The receptionist stated it was an error. I was told they had to contact my medical insurance to see if this procedure would covered and that would take several days. They told me I could not have the procedure until it was paid for and I miss my appoint ment time. Well, I missed my appointment time due to their errors. They kept asking me would I like to make appointment and I stated no they asked me again and I said no. I had to take off from work for this appointment. I asked could I be refunded my money and they said they only could give me back the money that I paid for the procedure. I asked about getting my $80.00 back due to me buying a product I didnt need. I was told they can due anything about that. This company seems to out for only money. I had been told I have cavities even though I was going to the dentist regular. I dont believe I had cavities. I have been told by other coworkers they switched dentist because of the same issues regarding false cavities. This company needs to be investigated into there billing practices. I am filing this complaint because I want to be reimbursed for the product they recommended and that I did not need. Also the staff their needs additional training. I have so many other bad experiences with this dental company I can explain. They also sent me a letter saying my daughter has additional dental insurance coverage and benefits that I may want to schedule and plan on using in 2024. On January 8, 2024 I visited Dental Associates for a cleaning and exam. I paid $999 for insurance that covers exams and cleanings. After the exam and cleaning they said I owed $155.20. They are claiming the cleaning was some sort of maintenance not covered by the insurance. I didn't ask for that and was not told that there would be an extra charge. After I paid, I made several calls and on January 31 finally was told that I should not have been charged, but still have not received a refund. I'm tired of calling and not getting results. I also feel that this is fraud and that many other people may have had this happen to them. I just want a refund, but would be happy to testify in any legal action.Customer Answer

I had tooth extractions on 8/29/2023. That day I was forced to submit a check for $3513 before they would start the work, even though the estimate I was given was $1245. That same day, after talking with *************************** at ** - she sent me a sheet showing that indeed I was overcharged. I was told by ********* that she could not issue a refund until the claim was closed. I received an *** from Delta Dental on 9/10 showing that payment to Dental Associates(**) for 8/29 would be made on 9/13. Online at ** site I saw that the claim was closed on 9/16 and that ** did not process one adjustment of $405 made by Delta Dental. The *** showed that the total patient cost was $1881 - not $3513. The ** site showed they owed me $1227 but actually they owed me $1632. On 9/21 I contacted ************************* and ********* at ********************** regarding the missed adjustment of $405 and the request for a refund. ******* said they would send the claim back for review. No one else responded. After a week of no response I went to the ********* office and spoke with *********, showing her the error with the claims processing. After a week this piece was resolved - my online account showed that I was now owed $1632. on 10/8 I received my refund of $1632. But I am lost over 3% a month on the $1632 and I want that paid to me. I also want compensation hours of time spent trying to resolve this. ** repeatedly overcharges for services and does not issue refund unless asked. I am working another issue where they overcharged me. They request payment upfront for an exam /cleaning covered 100% by insurance ** will not provide 2 dental exams a year when the insurance covers it. I have tried to schedule an exam but on the day of the appointment they refuse to complete it.Business Response

The BBB complaint was for Dental Associates overcharging me for services on 08/29/2023. *************************************** is working with me in response to a recent email I just sent regarding errors found for 2 OTHER DATES OF SERVICE (detailed below) for which I was overcharged. SHE IS NOT WORKING WITH ME ON ISSUES FROM THE *** 08/29/2023. Again, Dental Associations has refunded the $1632.00 due me from 08/29/2023, but there is the issue of money lost due to 2 months of interest lost on the money and the extensive hours I spent trying to get the issues from 08/29/2023 resolved.Because the experience from 08/29/2023 was so horrendous I decided to go back and audit to other Dates of Service (***) where I felt I could have been overcharged. I did find 1 other *** where Dental Associates failed to submit charges correctly to the insurance company, failed to apply adjustments per their contract to charges on my claim and charged me the full amount. ********* informed me that a check has been sent to me for that ***. There is another *** where they failed to send me a $85 refund that showed to be due me on their systems. When I asked for the refund they denied there was any due me. ********* has not yet agreed with me that they owe me $85 for the 2nd *** although the documentation I sent her clearly shows that they do.

Last year, I had upper dentures put in from Dental Associates located at ***** West Burleigh *********, Wisconsin. After having the dentures for just a few months, one of the teeth fell out. I had the tooth replaced in June of 2023. Shortly, after having that tooth replaced, another tooth broke and fell out. In October of 2023, I returned to Dental Associates in ********* to have them repair the tooth because it is obvious that the dentures that were made for me are flawed and defective. Dental associates refused to correct the issue. They stated that i am eating improperly and that I would have to pay additional money if I wanted to have my dentures repaired. I am a 79-year-old male whose prior dentures lasted for over 20 years without any issues. Rather than correcting the problem with the dentures, they used the absurd excuse that I don't know how to properly eat. Overall, I just want my dentures repaired or replaced.Business Response

On monday 7/17/23 I called Dental Associates because all weekend I had a very bad tooth ache.Was taking pain med,s so I could make it to monday. Told the girl , she said appoints were into Sept already.but seeing it was because of a bad tooth ace she would see what she could do, she got me in for wednesday 7/19/23/ I thanked her and went in for 8am on the 19th. Iam very stared to go to a dentest and they no that. So they take alot of xrays, I tell the girl what tooth is acheing , she says *** there for a full exzam , then the dentest comes in check my teeth , witch I know need work. He tells me what tooth is bothering me, witch I already told him what tooth it was on the left botton. He starts talking about my top teeth, that I should have them all pulled .Gets up to leave , He says ****** get a call for a pointment, I said for the tooth that is acheing me and she said no for the top teeth.I told them we live on social security no savings , sent me to billing they gave me a estament for upper denture $**** to ****. treent for the tooth that is acheing $500 to **** after I buy CAREPLUS DIMONDND PLAN FOR $105 or PLATINUM PLAN FOR $687 They said they each pay the same for the acheing tooth . I also told them we live on social security and can not afford all of that .Iwanted till Friday the21st. called them because my tooth was acheing more and wanted to know why they have not called, I did not want to be in pain all weekend. I told her about my appoutment I needed help now , she came back to the phone and said a nother dentest would see me that he was with someone but when done would call me . Iwanted all day , no one called , tryed saturdy , there closed. Had to call monday the31th. , asked for the administater, and was told I would have to pick up a form to make a complant, ******* needed to talk to someone now , she got me a manger, told her what was going on , she talked to the dentest , he said he could give me some antivotic so the pain would not be so bad untill they could get me in .they want over$500 just to pull the tooth now I already payed them $105 for insurance plus $352 for the exzame. *********** retired, so I called to book dental appointments for the whole family with ********************* in ******. She was listed as a preferred provider for United Concordia. Most of the appointments were just for prophylaxis which is covered by insurance at 100% including x-rays, cleanings, and evaluations. I explained she was in-network for us and I wanted to schedule a time when we could have multiple appointments on the same day. Even though I explained she is an in-network provider for us, the children were booked with somebody different without my knowledge. They refuse to acknowledge her affiliation with united Concordia who has her as a preferred provider at both clinics in the area. Although I was seen by *********************, they refused to bill as such and billed as a dental associate group instead of *********************. The services that should have been covered by insurance that weren't even performed by a dentist; cleanings and x-rays, were billed in full as dental associates. I complained previously, but didn't receive notification of any decision. In retaliation for my complaint, they reversed one of the insurance payments to increase our bill. Since Associated Dentists don't want to recognize ********************* as having a negotiated contract with United Concordia, none of the agreed cost for services were recognized, so we were charged full price instead of the price ********************* agreed upon when she became a United Concordia Preferred provider. This business is corrupt and dishonest billing strategy. We drove down twice, 2 hours each way, to have services rendered by an in-network provider to be over-charged and they never even cleaned by older sons teeth. They said there wasn't enough time to ex-ray, exam, and clean his teeth since he was 14 years old and needed a different type of cleaning. If they had been transparent about this, I wouldn't have wasted 6 hours of his school day to drive him down. This is a bait and switch method of recruiting patientsBusiness Response

For your reference, details of the offer I reviewed appearI did talk with dental associates again after filing the complaint, but they refuse to acknowledge that ********************* had a contract with United Concordia to provide services at a negotiated rate. They purposely scheduled my children with an out-of-network provider so that the prophylactic treatments were not covered. They have charged ** thousands of dollars for services covered at $100. We have already paid more out of pocket than we should have been responsible for. They even submitted my services with ********************* multiple times and under different claim numbers for the same services. They denied the insurance payments and the agreed providers job. They last submitted my claim one year after the fact so they would show up as out of network. This is fraud. Since filing out formal complaint has only been successful in raising our bill 1 year after we were actually seen, we have no choice but to proceed with arbitration.

I entered into a Ortho contract for $4200 approximately 2020. I made payments in the amount of $1208 and $900 from Care Plus which totals $2108. The remaining balance is $2092. However they are saying I owe an extra $908 and this is not accurate. The Administration Assitant ******* keeps ignoring my phone calls and refuse to help to resolve this matter. I do not want to pay for charges that I am not responsible for. Can you please look into this for me Im tired of being over charged and having no recourse as a consumer.
