I called to schedule appointments for my family to see ********************* from ********** Dentists because she was listed as an in-network provider. ********** Dentists only scheduled myself with ************** and had the children see a different dentist who was not in network without my approval. We drove over 2 hours to see an in-network dentist, and it is my belief they purposely switched the providers to ****** more money from us. The children had x-rays and prophylaxis treatments, which should have been free as part of our insurance benefits. I received an Explanation of Benefits from my dental insurance about the charges I incurred and the negotiated rates, but ********** Dentists claim they do not acknowledge her affiliation with United Concordia and so refuse to adjust any costs to the contract agreement costs. We were charged $3,138.00 for our dental care. I even purchased their supplemental insurance to reduce the costs of *****'s fillings that were $1522 before any adjustments. The fillings should have been at the in-network rate and adjustment for a cost of roughly $684 before the supplemental insurance credit of $460 was deducted. I have already paid $430, which is more than would have been due if our appointment wasn't changed to a non-participating provider. It is poor business practice to have a ***** of ********** Dentists with central billing and disparate insurance acceptance or acknowledgment. I have attached the bills showing what was covered by our insurance. I also have included insurance documents reflecting some of the care was a participating provider and some was not. Date of first visit 3-9-2022 x-rays and exam Date of service 3-31-22 Insurance was provided on date of first service 3-9-2022. No pre-authorization was done. **** sent to me for full amount. Over the course of the next few months the i left multiple voice-mail in billing department general mail box. 0 response. Emailer the clinic finally got a response. They allegedly call my insurance and were told no coverage at time of service. I called my insurance confirmed I did in fact have coverage and forwarded that to ********************* ***************************** 0 response 3 days letter I get letter from collections. Dental associates received two more phone calls I was told supervisor would call me. Collections ****** tells me I have to submit claim and pay them the difference. Still no response from dental associates about filing claim as they should've. I can provide email chains and communication sent. Absolutely unacceptable response from a business. I have been trying to pay my portion of the bill but they refuse to contact me about doing what they were supposed to do in the beginning.Business Response

In Novenber of 2020, I went to Dental Associates in ******** ** for procedures to have my broken permanent bridge of 4 teeth removed, a tooth extracted and implants placed for contructing an implant bridge. I was quoted and paid $13,543.20 for all services including implants and bridgework. We refinanced our home to pay as advance payment was required. We are in our seventies. We were verbally assured at time of payment that all services were covered by this amount and receipt (uploaded) states "Paid in Full for Implants on 11/19/2020." I had also paid an additional $1500 on a Care Credit account for the extractions so total paid was $15,043.20. 4 implants were placed but only 3 were successful and one removed. The pandemic and closings delayed my procedures until Nov. of 2021. Before the bridgework was started, both I and my dentist, Dr. *** had verified that I was paid in full and did NOT owe any more money. The first bridge was not made correctly and did not fit so a second one was made. I hope I was not billed for their mistake. In December of 2021, I was billed by Dental Associates for an additional $4540.70 (later reduced to $3280 after insurance payment). The bill is for implants but I have already paid in full for implants as on my receipt of 2020. The same fees for implants they are billing now are identical to the fees I already paid on my treatment plan. I called the billing office many time at ************ and discussed this but no one would discuss my case or review the charges! The billing manager would not call me back although I requested her to. In August, they sent me a letter threatening to send me to a collection agency and, since I have good credit and do not want it tarnished, I had to agree to a payment plan. I have gone through a lot of pain and suffering with all these procedures and now feel I am being double billed and overcharged. Please help find a resolution! Thank you!Business Response

I have reviewed the response made by the business in reference to complaint ID ********, and find that this resolution is satisfactory to me. The manager of the ******** Dental Associates contacted me my phone this morning and offered a very satisfactory response to my problem and I am very grateful for his help. They worked out our billing problem and offered a sincere apology and very acceptable solution. We will continue to use Dental Associates for our dental needs as they have a very caring and competent staff. We would highly recommend Dental Associates to anyone needing dental services of any type!

5/19/2021 was the date of the crown. From the moment I chose the material for the crown. I was bullied to try to change my mind about it. A week before the crown I was charged for a core build up that just fell out two days before the crown. Everyone acted like oh well but as far as I could tell that was a waste of money. Now a little over a year and it comes off and has a hole in the center of it. Now I may not be a dentist but this goes against everything I can ****** or ask anyone. I also do not have dental coverage at the moment like I did when I recieved my crown. Now I have to walk around with my exposed ground down tooth until I can figure it out. I don't even want my money back. I just wish they would stand behind their work.Business Response

[I only filed this complaint after I was basically told to get bent last night after she "checked with her manager". Now because of this complaint they are willing to look at my tooth free of charge. I shouldn't have had to do this for that. I will see how Tuesday appointment goes and I will follow up.]

In 2021, I was told by my dental clinic, ****************** clinic in *******,, Wisconsin,, I would need to have my wisdom tooth extracted. Partnership is a dental clinic for low income patients. They gave me a referral to a surgery clinic in Oshkosh, nut I could not afford the $650. I started calling other surgery clinics, but the average cost was $600-$700.I stopped at Dental Associates in *********** and talked to **** at front desk. She checked and said ******************** would be in my insurance company in 2022, and my cost would be $49, but I would need a consultation. I agreed and had Partnership send the Bitewings and referral, and had the consultation Nov. 12, 2021. and paid the $96. fee. I was told again by *********** would pay $49. for the extraction. In March of 2022, I stopped at Dental Associates and was told by **** ******************** was in my insurance, and had her check again my cost and she told me again, $49. She said there was an opening April 5, and I took it.April 5th I came in for my extraction and had **** double check what I would have to pay, and she assured me it will be $49.******************** said the extraction was easier than he thought, so it didn't take long. I paid my $49 and left.More than 4 months later, on August 20th, I got a bill in the mail for $667. for the extraction. I was totally shocked.I am retired and low income. When you're told numerous times how much you'll pay, you believe it.I am disputing this bill, want this bill dropped to zero, and do not want this to affect my credit score.Business Response

When we started orthodontic care for my son, there was a stable provider there, ****************. When she left, a few months into care, there were times we couldnt get in for an appointment in a timely manner. They also kept changing their plan of care from Dr. ******* initial recommendation, as there were a variety of, if at all, orthodontists coming in to cover care. At one point, my son came out and said they had shaved his teeth down, when that wasnt part of the original plan and no one talked to me about it before doing so. There was no communication between the providers and parent once **************** left and I didnt approve of this. Due to lack of care, constant change in treatment, and difficulty with accessing care, my sons treatment was prolonged. We relocated and I was asking for a refund as we werent done with treatment yet. I have decided to leave the organization as I dont feel like their care was appropriate and they stated I will not get any type of reimbursement as he was not Debanded. I have asked for calls back and no one has returned my call, often sending me to another department to get no where.Business Response
