      After recently reviewing my credit report from your company, I noticed several inaccuracies that need to be addressed. The accounts in question are listed below:Company Name: TBOM/FORTIVA Account #:****************Balance:$602.00 Under the Fair Credit Reporting Act (15 U.S.C. 1681g), I am formally requesting copies of all documents in your records that were used to verify the accuracy of these accounts. Please provide these documents in your response to this letter.I respectfully ask that you do not reply with a generic statement claiming the accounts have been verified. Instead, I expect to receive the actual records supporting this verification. If no such documentation exists, these disputed accounts should be removed from my credit report immediately, as required under Section 611(a)(5)(A)(i) of the FCRA.Including unverified or inaccurate information on my credit reportand continuing to share it with third partiesunfairly damages both my credit standing and my reputation. I ask that you take this matter seriously and resolve it as soon as possible.

      I Check My Credit Report Daily and my Score Dropped 42 points today because of these 2 accounts:TBOM/********************** There Accounts were Closed last Year with a $0 Balance They were undated incorrectly and I want them Updated to Paid as Agreed and 100 % payment on time. 15 U.S. Code 1666b I want them Corrected immediately.This is stopping me from ***************** freedom and causing unnecessary Stress I have attached proof

      04/29/2025 I called milestone credit card services twice and requested to have statements for October 2024 through February 2025 mailed to me. Well they said they were going to mail them but they did not do it. I am still waiting for them. So I called and they refused to mail the Statement. I am tired of it. They even took the Fortiva Mastercard account off the ********************** report. Why. There Is A balance on the account. They are cheating me **** I don't like it.

      Okay they go by Imagine Credit Card but same address. They dont have customer service number and you cant message them but Feb 2025 i was charged ***** late fee but my payment posted day due then today my time 3:45pm make a payment that will also be paid on due date not a day later. They dont give any other option to pay with debit card but you cant fill in card number. I want my late fee back and to be sure no more fake charges come along. I now know kinda a shady company with no customer service available except to send letter, like years ago. Please help me I really should have read all about company before excepting card. But I refuse to pay late fees when I take the time to make sure paid on time.

      I am writing to report a fraudulent account that has been opened in my name with TBOM/ATLS/ASPIRE. I am a victim of identity theft and did not authorize the opening of this account. Below are the details of the fraudulent ******************************** Name: TBOM/ATLS/ASPIRE Opened Date: 7/1/2023 Account Number: ****************High **********************: $0.00 I kindly request that you close this account immediately and remove any negative information associated with it from my credit report. Additionally, I would appreciate it if you could provide confirmation of the account's closure and any relevant documentation related to the fraudulent activity.Thank you for your prompt attention to this matter.

      I am not liable for this debt with *******. I do not have a contract with *******. A vivint sales representative came to my house selling their services. Vivint took my information and opened a line of credit through ******* in my name without written or verbal consent. I reached out to ******* for the documents they used to run my credit and open a line of credit, and they were unable to locate anything. Attached is the original contract from ****** and nothing mentions running credit or *******.

      I was asked to pay for an Aspire Credit Card I never opened. When I called their phone number, the automated system confirmed my card wasnt even activated. I got a representative on the line who wasnt professional and told me not to worry about the balance owed and that the account was ************, I got an email saying I have to pay. This was an account I never opened with a card I never activated.

      Sometime in March 2024 I went to Ideal Image located on ****************** in *****************. For the purpose of an esthetic resolution The staff named "****" upon evaluating what problem I was there for informed me that I could get my problem resolved on a payment plan. She then via computer entered my information in the computer and told me I had been approved for $5000 and she scheduled me for my desired procedure. I changed my mind and called Ideal Image and advised them that i was no longer interested and would not keep the upcoming procedure. I then called Fortiva. and advised them of the same. I was told that my account was closed. However,I am now receiving statements of a past due payment although I canceled the appointment before the scheduled appointment date. I realized that this company along with *********** were both fraudulent businesses. I am now being told that this transaction will be reported to the credit bureaus
