      hospitals nudge patients away from financial assistance and towards higher cost financing programs. *****, is an example that hospitals employ to misdirect patients away from charity care. These include (1) presenting medical payment products as a financial assistance. I wasnt aware of other options. Now ***** has illegally reported a medical debt on my credit reports to all reporting agencies. Thus, as other complaints I have never been able to login to the website, nor have I received an invoice for services. I will be sending my complaint to the the secretary of the state department and to the relevant authorities to have my credit report cleared of the ***** I did not receive any medical treatment. I only took my neighbor and child to be treated at the hospital. Was there five hours and the child was given a pill for an upset stomach. I got a bill by signing a credit contract for 1400 plus dollars and a past due bill and late charges.

      This credit card company has reported a late payment on my credit report from December 224 to my transunion credit report. I have disputed this 3 times with ********** and the dispute has been in my favor and the late payment has been removed all 3 times. ******* keeps adding the late payment back to my credit report even tho I have provided proof I was never 30 days late! I have even offered to settle with them and my account is closed and they have refused! They keep charging monthly fees and maintenance fees! I want this late payent removed off of my credit report once and for all and for this company to stop adding it back when all 3 credit bureaus have ruled in my favor.

      05/10/2025 I requested statements for October 24 through February 2025. They said they were going to send them and now they are refusing to send them. I need them sent as soon as possible. I owe ******* and I need to see them. They are not online for the Fortiva master card. They are giving me t he round around.

      This company routinely reports accurate information to the credit bureaus. And is accruing unreasonable, unwarranted and undisclosed repeated charges completely inaccurately keeps up with the balance owed. None ofthe numbers add up to balance owed. This company is running a very fraudulent company. Praying on Americans.

      After recently reviewing my credit report from your company, I noticed several inaccuracies that need to be addressed. The accounts in question are listed below:Company Name: TBOM/FORTIVA Account #:****************Balance:$602.00 Under the Fair Credit Reporting Act (15 U.S.C. 1681g), I am formally requesting copies of all documents in your records that were used to verify the accuracy of these accounts. Please provide these documents in your response to this letter.I respectfully ask that you do not reply with a generic statement claiming the accounts have been verified. Instead, I expect to receive the actual records supporting this verification. If no such documentation exists, these disputed accounts should be removed from my credit report immediately, as required under Section 611(a)(5)(A)(i) of the FCRA.Including unverified or inaccurate information on my credit reportand continuing to share it with third partiesunfairly damages both my credit standing and my reputation. I ask that you take this matter seriously and resolve it as soon as possible.

      04/29/2025 I called milestone credit card services twice and requested to have statements for October 2024 through February 2025 mailed to me. Well they said they were going to mail them but they did not do it. I am still waiting for them. So I called and they refused to mail the Statement. I am tired of it. They even took the Fortiva Mastercard account off the ********************** report. Why. There Is A balance on the account. They are cheating me **** I don't like it.

      Okay they go by Imagine Credit Card but same address. They dont have customer service number and you cant message them but Feb 2025 i was charged ***** late fee but my payment posted day due then today my time 3:45pm make a payment that will also be paid on due date not a day later. They dont give any other option to pay with debit card but you cant fill in card number. I want my late fee back and to be sure no more fake charges come along. I now know kinda a shady company with no customer service available except to send letter, like years ago. Please help me I really should have read all about company before excepting card. But I refuse to pay late fees when I take the time to make sure paid on time.

      I am writing to report a fraudulent account that has been opened in my name with TBOM/ATLS/ASPIRE. I am a victim of identity theft and did not authorize the opening of this account. Below are the details of the fraudulent ******************************** Name: TBOM/ATLS/ASPIRE Opened Date: 7/1/2023 Account Number: ****************High **********************: $0.00 I kindly request that you close this account immediately and remove any negative information associated with it from my credit report.
