      If only zero stars were possible! To make a long story short, I was told by the guardian rep that came to our employment, by my HR rep, by 2 different representatives i spoke with on the phone that Invisalign is covered, that i have a copay of ****. This is a flat out lie. After 16 phonecalls back and forth with them and my orthodontist, They do not cover Invisalign.! If i call right now again and ask they will tell me its covered, thats theres no age limit and i dont need pre-auth. Lies. It is not covered and i changed my insurance because i was told it is covered, had to switch my primary dentist and waited the necessary year guardian claims is needed to be eligible for aligners. I could gave been paying the same price w my old dentist and started treatment a year ago. This is disgusting.

      The Dental is trash they hired a dentist to deny claims they said was a practicing dentist which was a lie after finding his name he had sold his practice and did not practice dentistry at all. He just reviews and denies claims for Guardian, so they letter they sent is a Lie. he works for them and not a practicing dentist at all. they wanted my dentist to drill out the tooth and take pictures to see if cracked tooth syndrome applies and they may or may not cover it at that point. that is a ridiculous thing to request they will deny it and say it doesn't qualify. these people are no more than a Scam i wish I could not pay my premium or get a different dental insurance, but these clowns are a bad joke and should be investigated for fraudulent business practices. Zero stars left

      This company has terrible customer **********************. I had hip replacement surgery on 9 May 2023 and have been waiting on short term disability payment. I was notified by mail on 22 May 2023 that an investigation was pending and all my doctors needed to complete a questionnaire. The questionnaires were sent and received on 13 June 2023. Their policy is that it takes 10 business days from receipt of information. However, Guardian faxed a request to only my surgeon on 20 June 2023 but did not inform me of any actions via text message as they stated they would. I called Guardian on 23 June 2023 and learned they faxed it to a number that did not exist. I gave them the correct number that day and they received the form back that day which started the 10 business day decision clock over. On 10 July 2023 the end of the 10 day decision period, I called and was told it was escalated up to the case manager who would make a decision in two days. I called on 13 July 2023 and learned Guardian had faxed a questionnaire to another doctor. I discovered it was faxed to the wrong fax number again. I gave them the correct number and Guardian stated the 10 day period would start again once they receive the fax. Guardian is delaying with these requests which can go on forever. Further, I am supposed to have received a text whenever information is requested and that has never happened.At this point, I believe Guardian is trying to wait until I return back to work because they know my return to work day from the doctors required form.This is a terrible company. It seems there is nothing that can be done about it.

      Cancelled my policy without contacting me, I paid premiums for the months I had the work done, dentist said insurance was all good. Cancelled policy without paying dentist and wont reinstate my policy. Shady company, ill take my business elsewhere. ***** hours to get a call back from supervisor, never received one.

      Your CSR'S don't have a clue, I signed up for insurance with your company and have had the same dentist for 20 years . They have been in network with my group plan and my Cobra plan. When iI sign up for an individual plan low and behold they are not in network. So I ask for a list of network providers in my area and it shows them on the list in network. I am talking to another of your CSR's and am looking at the list but am told they are not on it. So I guess I am seeing things. Thanks for your dishonesty and I hope you have a blessed and wonderful business. The worst customer service and company I have dealt with since Big Lots!l

      Trying to get insurance with them through my employer. Kept getting emails saying they needed evidence of insurability to get the process started. Their website is a mess and their automated phone system is even worse. Seeing all these 1-star reviews doesnt inspire confidence either. Thank goodness I was only looking for partial insurance through Guardian. I cant imagine having to deal with this nonsense on a regular basis.

      As a dental provider, Guardian is an awful insurance company to work with. They do everything NOT to pay a claim. You can never speak to anyone that isn't reading off a script. If you're shopping around for dental insurance, I would highly recommend looking for other dental insurance. Even for dental offices, they are very difficult to work with, and will deny your claims and leave you with a huge out of pocket!

      No one should ever spend their money with this company. I took a leave at another company and it was much much easier and quicker than this. On March 6th, I decided to call just to see if my recent ADHD and anxiety diagnosis was eligible for a paid leave. The agent told me it was, and told me that they could put in the leave request now if I wanted. I told the agent that I hadnt spoken to doctor yet, and that I wanted to do so soon. The agent proceeded to put the request in and advised me to continue working until I got the paperwork. 10 days later, My HR rep at my job advised me that was incorrect, and that youre supposed to be prepared to leave the day you put in the request. That first piece of misinformation has made this process a living h*** My ACTUAL leave was from March 14th-April 18th per my doctors recommendation. I was consistently getting treatment and paying for sessions and medicine every week on my leave. I was not faking it, I was getting the help I desperately needed. This was all in the paper work they received for the claim. However, now theyre saying because I put in the request on the 6th, my leave would only be approved until April 5th. In order to correct this, theyre asking me to send THE EXACT SAME PAPERWORK MY DOCTOR ALREADY SENT. Another rep I called 3 weeks later, said that the person running the case could just use the papers I already sent, which made sense to me. But today, I learn that I was lied to again. It is insane to me how I have to **** my doctor down to have him put the same exact info down. It has not been easy getting in to see doctors lately. I have no words. On top of all that, the workers have been rude, dismissive, and they expect me to be the most professional in the conversation, yet Im the one with the mental disability seeking help from you! You should be ashamed of yourselves and you need to train your phone agents a lot better than this!

      LAWYERS:Collected a premium for my daughter for 3 years but never activated her insurance. During that time was informed by Guardian she wasn't insured I made decisions for care based on the notion she was not covered when she was. They made my life h*** trying to get reimbursed for dental work, resubmited docs over & over again. We forced our Insurance broker to fire them as the Dental Ins. provider, had multiple staff members experience this issue. They turned around & billed my staff for the last month of services after they were fired by the broker, turned people into collections all because of their errors. Insurance fraud lawyers hit me up or investigate this horrible company fair confident it'd be an easy pay out.
