I paid for a crown in advance it was not able do be done. I asked for a refund several months ago many times! I believe that the first request was about December! I have talked to several employees at the office and they have sent requests to their higher *** with no resolution. I am a senior on a fixed income and this has gone far beyond the time I should have received my money back! I believe now they should be paying me interest on the amount of money they have held so long! Thank you for your help in this matter! Date of service: 12/22/22 Amount charged: unclear Business committed to provide: periodontal splint Dispute: periodontal splint fell off within two months; the periodontist who performed the procedure was no longer in the office to replace/fix, nor was there any other periodontist in the office or in the Perfect Teeth network who could/would replace it. I've been seeking a refund since Spring of 2023 as I paid soon after the date of service once my insurance claim was processed. Since this time, I've gotten bills for the service I had already paid, been told my balance was at zero only to be sent additional bills. I have called and emailed both the billing department and the Monaco and ***** office to which there has been no resolution or clarity around these additional bills or the refund. The last letter I received said this was their third attempt at contacting me, even though I have sought out resolution and received none.Initial Complaint

Subject: Request for Refund of Treatment Payment I am writing to address a matter regarding the treatment services provided by Perfect Teeth. On January 25, 2022 I paid $6,306.00 for Comprehensive Orthodontic Treatment. My treatment was going okay for about a year until the practice abandoned my treatment. Following my last appointment at the office in May of 2023, I have encountered numerous challenges in attempting to continue my treatment.Despite my efforts to schedule appointments throughout the year, I have been met with various obstacles such as the unavailability of an orthodontist, technical issues with the scanning equipment, and requests to settle my account before further treatment could proceed. My attempts to communicate via phone have gone unanswered, leaving me with no option but to visit the office in person, only to be informed that orthodontic services were not available on-site.Regrettably, my most recent appointment was abruptly cancelled, and upon inquiry, I was informed that the clinic currently lacks access to invisalign treatment indefinitely. As a result, my teeth have shifted back to their initial position prior to commencing treatment, causing me distress and frustration. I am devastated that I have wasted time and money in this practice which is a scam.Given the circumstances, I am now formally requesting a refund of the payment made for the aforementioned services. I have been going here for about 2 years for Invisalign. During this time the did not set up my payments correctly which resulted in me going to collections. They told me I was done with Invisalign although I asked to keep going because my teeth were very crooked, they proceeded to blame it on the orthodontist they fired. Tried to over charge me after stating I didnt owe anything. They have continuously not booked my appointments after saying they did. Canceled my appointments without letting me know. Left me 6 months without a retainer not allowing me to get another or schedule an appointment just left me with the buttons on my teeth. They never answer their phone so I cant discuss with them my issues unless I drive 45 min to their town. I have asked for a refund for over a year any they just tell me they will fix the problems but they dont. I just want my money back so I can go to an orthodontist that can actually see me and fix my teeth. I know they have not kept good records of any of this because every time I go in they say there arent any notes. I took some notes in my phone about it though.Initial Complaint

I have been attempting to resolve a billing issue with this office that began in November of 2021. I have always paid for all services at the time of treatment with one exception when there was no one at the front desk. I quickly paid for that service shortly after treatment. Due to a billing error where a treatment was incorrectly assigned to the same tooth twice the claim was rejected by my insurance carrier. This was the start of my nightmare where if I was even able to get someone from the office on the phone, maybe one out of 10 calls were answered, they were unable to help let alone understand and solve this issue. After being told numerous times that the issue was on their end and I had nothing to worry about I began receiving past due notices from their collections agency. There is no other number to call where I can be confident that someone will answer and it has been 14 months of trying to correct this error with my local location. I am willing to pay the amount being asked simply to be done with this situation but until I have exhausted all other options I am unwilling to do so. I will NEVER return to this location or any of it's partners and simply want the issue resolved so that I can move on and never think about them again.Initial Complaint

I was seen a few times in March , April may and June of 2023. I needed to have a implant done and was concerned about the cost as I don't have a ton of money and so I asked to put money in my account ad a credit to apply when I needed to have the procedure done. I put down I believe over *******. Then I ended up having a tooth that they could not numb to extract. I had to see a different specialist and then my perfect teeth dentist was on vacation for a month. I called back to request my money be refunded and was told they would send a check out in 10 business days. I never got. Check. I've called back over and over over the last 6 months and no matter what time of day I call accounting is always about to go on lunch or they are gone or not answering. There is no manager. My son committed suicide and I want the money to pay for his headstone and I feel like there is no way that over about 20 phone calls throughout this time no one is ever available. Friday 11/17/23 4pm: I was supposed to have my bi-annual cleaning appointment. I drove 30 minutes down to the office after leaving work early to get to my appointment on time. After parking, I walked up to the office to find it completely locked with nobody inside. I called the phone number and was transferred to the customer service line and was told the ********************** was closed that day. Monday 11/20/23: I called the office and asked for an explanation. I was told that the office has been closed on Fridays for 3 months. I asked why nobody had reached out to me and why I was still sent an email confirmation. They had no response. I asked if I could be seen before December 1st, as I was having a major surgery (which is why I had my original appointment scheduled for when I did). They put me on a cancellation list and said theres a very good chance of me being seen by December 1st. Monday 12/11/23 11:18am: My sister received an unprofessional text message informing her that her upcoming appointment for Monday (12/18) was cancelled. Tuesday 12/12/23: I called the office again. I asked why I had not been contacted in the almost 4 weeks since being put on the cancellation list. I was told that the list is very long. I asked why was I told that I had a good chance of being seen by 12/1 if the list is actually very long. No response. I asked if my sister can get re-scheduled before the first week of January. She goes to college in ********, and she had scheduled her original appt for her winter break. They are booking out until April ****. They told me if I call the office on 8th Street, that office would be able to get us both in on 12/18. I called that office, and they are booking out until February ****. This Perfect Teeth office (***************************************) is extremely unprofessional and lacks basic communication. This instance has been the final straw of many since 2015.
