Just received a bill in October for teeth cleaning in March.Just said my insurance was maxed out. There was No Indication at the time of service.Did not complete the Services billed for. They claimed to to have done X-rays and other services. At the time of appointment it was total chaos at the building. No Dentist on site.No reason for billing so late. Very rude people first calling the telephone number on the invoice. Then I had to go back to the Green ******** directly ( No number provided) . Very rude people all she had to say was she wasn't there at the time.Initial Complaint

I cancelled my order for gumx over the phone and they said yes to cancellation. A few days later i get a bill for it. I called to ask about it and the MANAGER ****** said that there was no cancellation and that she never does them. I asked if phone calls are recorded and she said yes. I then said that phone call was all i needed to prove my cancellation. She proceed to say i cant retrieve that call and try to commit fraud by saying over and over that i did not cancel. She refused to give me customer service number. I called the number listed online and it doesnt work. I called back and spoke to someone else and they found the cancellation on my account. They said to not pay the bill i received until they resolve the problems with the cancellation. Now im being sent more bills with INTEREST. Its nearly doubled. No refund for the canceled items either after months.Initial Complaint

October 25th I received a letter in the mail about a bill with your dental office information, billing me for $334, with no breakdown of why such a high charge. First of all, I have ********* Second, prior to visiting, phones calls on setting up the appointment and at checkout, I was told my ******** is accepted. Third, your patient billing is done through InstaMed directly on your website. I am getting this bill through a complete different billing portal asking for money. Does perfect teethe work with a scamming entity to receive chunks of the so called bills that should be billed to insurance. I amo not the first one to with this specific office to have this same issue. I went October 26th to the office to speak with the receptionist and she specifically said I should not be getting billed directly if I have ********* She took a copy of my letter and told me she will contact me on research. It is now November 1st and I have not heard back. This letter that seems to be a scam better not be sent to collections.Initial Complaint

I scheduled a routine dental cleaning and checkup on 10/03/2023. After arrival I joined their dental program for discounts on services. After consultation the dentist said that I needed a "Deep Cleaning" and that there was an upcharge. I told them that my reason for the appointment was for the cleaning, and I really wanted to do it. It took only five minutes using the conventional tools that the hygienist normally uses to clean my teeth over the past 50+ years. I would definitely consider it a routine cleaning that normally costs about $100. They charged me $500 on top of the $129 for the dental program. I complained to the hygienist about the cost and left. I called the office when the staff returned from their lunch break and spoke to the office manager. I told her that I felt that they had upsold me a service that they didn't perform and lied about the actual need for the service. She said that she would refund $250 dollars to my credit card. I have called and spoken to her four or five times since then and have not received the promised refund. I feel that they owe me the entire amount and should then charge the usual price for an ordinary cleaning. I have had my teeth cleaned by hygienists my entire life and they have spent twice as much time completing the cleaning and never tried to overcharge me like this. This office is notorious for trying to upsell me on unnecessary services and I never have agreed to them. This time they just decided a different approach and overcharge for a normal service. I will not return to this branch. I have tried to find a corporate office to submit my complaint but can't find one since they are independently owned and operated. Have received a string of statements from Perfect Teeth for the amount of $344.00, but no additional information. Have called numerous times to try and get some information about this, but either never get in touch with anyone, get hung up on, or get in touch with customer service and they can't give me any details for why I suddenly owe this amount. I'll note that this statement has been issued to me about 9 months after my last appointment with them, and I haven't heard anything from them during that time. On the final notice now, with the threat of collections. I'll be sending a letter to their corporate office asking for details on this statement, and suggest other do the same. You may not get an answer, but it's good to show collection agencies that you have made the effort to get ahold of them.Initial Complaint

I am in the process of getting dental implants, which first involves the extraction of all my teeth, then the fitting of some temporary dentures, and finally the installation of the implants. I was told by my insurer *************** Advantage) that they would cover the total expense for the procedure, and I would have zero co-pay. Cigna directed me to the Perfect Teeth **************** in ******. Before the extraction, Perfect Teeth failed to inform me that although ***** would cover 100% of the cost of local anesthesia, Cigna would cover 0% of the cost of the intravenous sedation, which is what Perfect Teeth decided to use. After the extraction procedure was completed, Perfect Teeth handed me a bill for $1,857 for the intravenous sedation. I am struggling financially and on a fixed income and was not prepared for this expense. Perfect Teeth just suggested I apply for a CareCredit loan to pay the bill. I believe I was manipulated into using a very expensive sedative, even though they knew that ***** would cover 100% of the local anesthesia option. So far, Perfect Teeth has ignored all my communications about this issue. I feel they should have clearly explained all my options beforehand and encouraged me to choose the affordable option. Im disgusted with the unethical billing practices of Perfect Teeth **************** in ******.Initial Complaint

Well, to start, this location put an implant in my mouth when I had an infection, and they should never have placed an implant. This is backed up by three other dentist opinions. That implant failed, and I had to pay to have it removed. This was over a year ago and I just got a bill from Perfect teeth about four months ago for another $155. The bill does not say what it is for and I have been trying to get in touch with that location to figure it out for the past four months to no avail. Now it has been sent to collections and I have no way to get it sorted out because nobody will return my phone calls. Ive called other locations and nothing is working.Initial Complaint

My son had braces put on in 2020 as part of a 2nd phase. In 2022, he should have had them off by end of year. The practice lost their orthodontist and had fill ins every month or every other month. The fill in orthodontist didnt make any changes, only provided maintenance and changes the rubber bands. The last appointment we had was in June and no one has contacted me. *** tried contacting them and Im told the only orthodontist is an hour away and of course the office is closed. You can never talk to someone there in the office. I need a refund on my sons orthodontics so I can go to another practice and have it finished. This office is horrible about communicating and being transparent with customers.Initial Complaint

Friday, June 14, 2023 (the fourth appointment attempt because their call center kept changing my appointment without consulting me), I went to Perfect Teeth Golden for a routine dental check-up and cleaning. Upon arriving, I checked iwth the receptionist that they accept my insurance (as I always do with a new health care provider). She said "yep, absolutely!" I had a terrible visit, where the dentist who "cleaned" my teeth scarped my teeth in three strokes and then said "your teeth are perfectly clean. Do you want me to keep going?" I informed I did want her to continue. After looking around and making two more scrape strokes with her scraper she said, "Do they feel clean now?" I informed her that I wanted a full clean- scrape all the teeth, get fluoride, etc. She did not scrape any more, but went and got fuoride. On my way out, the receptionist said I wouldn't owe anything. They then billed me- apparently out of network- suggesting that I owed them over $100 on Aug 19. I attempted to go into their business to speak to them four times over the next two weeks DURING BUSINESS HOURS, but they were always closed, despite claiming to be open online. You cannot reach them via phone. I sent a letter (attached) certified (attached) with my complaints and have received no response. Their link to the original bill I received on Aug 19 (so less than a month ago) has "expired" so I can no longer view the bill and what I "owe". I in the weeks since, I have driven by multiple times a week and the lights have been off every single time. They are unreachable, did not provide the advertized service, and lied about accepting my service.Initial Complaint

I am receiving monthly statements requesting payment of $186.00 for services Dec. 12, 2022.After multiple visits to the ******** Perfect Teeth office and communication with Operations Manager, *************************, in conjunction with speaking with a Delta Dental Representative (*********), it was determined that the office had not followed proper procedures and charged for planing. Attempts to have my numerous phone calls returned has been futile. The only way for me to speak with ************** is to physically appear in the ******** Office.Because I received notification from ************** that my account had been settled, I have not made additional payments. According to Perfect Teeth records, this balance is now 90 days overdue. I would appreciate it if this balance can again be reinvestigated and I will happily contact my insurance provider again to do the same. If, for some reason the account has been reconsidered that is contrary to what I was told by **************, then I believe it would be professional for Perfect Teeth to contact me with the reasoning and specifics of that decision. I feel that I am being harassed by Perfect Teeth and/or the ********, ** office with these automatic billing statements. In the event that they/you are sufficiently able to explain the reversal of payments being settled, I will begin a payment plan of $5.00 each month. Sincerely, *************************** Delta #**************
