      My daughter and I have been going to Perfect Teeth for numerous years. They handled our braces and our normal teeth cleaning, etc. They have not had an orthodontist onsite for over a year, so we have no way to follow up for care or concerns regarding our retainers or the two gaps I still have. I had to pay out of pocket for my braces and now will have to pay someone else to finish what Perfect Teeth is not completing. I was told today that I could be referred to another orthodontist in Peoria... which is at least 20 miles from my house. And if you know Phoenix Metro traffic, that could take me an hour one way. I chose the location at ***************************** and 90th Street for a reason!!I also had a cracked tooth filled by **************... he did an awful job. It has to be redone and he was awful. I was happy to hear he has quit. To top it off... today, before my daughter's cleaning, I received a call telling me I owed $25.60 for a filling my daughter had a few months ago. I never received a bill. The girl told me I was sent to collections!! AND I had to pay the $25.60 before the dentist would see my daughter today. WTH?? I have been with Perfect Teeth for so many years and paid them thousands and thousands of dollars to be told I was sent to collections on a bill I have NEVER received or a phone call to say I owed it. So I came her to read reviews and see if there was a corporate number for Perfect Teeth... the ************ number has been disconnected for their corporate office. How do I get my $$ back for my braces that I paid for fully since 1) the process is not complete and 2) there is no one to complete it? I am so disappointed. I have referred so many people here, AND NEVER received the $50 referrals. Now I am going to go through my account with them because I am sure I have been overcharged many times after reviewing all of the complaints. If anyone can direct me in the right direction I would greatly appreciate it.

      ACCT: 617Y4-0036053179 Complaint against *********************** (TSI), bill collection agency for Perfect Teeth Contested bill amount: $125.60 I've always paid Perfect Teeth in full before leaving the dentist office. Around Nov 2022 I began getting a bill for $125.60. As usual I assumed it was late reimbursement from insurance. I offered to pay the full amount several times while at the dentist, but they seemed content to wait for insurance to catch up. I paid the $125.60 and confirmed with them they received the money in January. They even admitted that they actually owed me $145 dollars. They sent me a check for $145 dollars in March. Yet every week for two months I have received threatening letters from TSI (a bill collection agency ) stating I owe them $125.60. Apparently the bill collection agency would rather harass me with automated threat letters than actually check to see what the account balance is .... zero. I've called Perfect Teeth multiple times and asked them to call off the harassment from ***************** TSI. No effect. Request Perfect Teeth and TSI contact me and confirm they are stopping harassment, and have not made damaging reports to credit bureaus.

      I was contacted by Perfect Teeth -East 104th **************************************************** in April 2022 and they were requesting payment for $48.10. I requested more information and I received more correspondence requesting payment. I attempted to obtain more information and specific details of the alleged debt. I left several messages and received no response. I finally had an employee answer the phone and she wouldn't give any information except to say that the amount due was form two years ago for my wife's treatment. I payed the $48.10. I asked the employee if there were any other charges. The employee said that there were no other charges. I asked for a receipt and any other documentation for my account. I never received anything further until December 2022. It was another request for payment for $307.20. I left another message asking for further details and noted to Perfect Teeth that neither I nor my wife have been patients in years. I received five more requests for payment and five more letters regarding request for payment. I left five more messages and did not get any more specific details that were pertinent to the request for payment. I also did more research about Perfect Teeth and found information about other people who had been ghosted with a demand for payment after years of no communication or service. This seems to be an ongoing issue for Perfect Teeth.

      More recently I had gotten two crowns and then several weeks later I got a notice to get an appointment for my cleaning. When I called there was a new person at the desk that didn't seem to know how to handle the question or request. She said the cleaning would cost $300. I knew that wasn't right and asked her to call me when they got it right.Then in the following weeks up to date 2/13/2023...today's date...I called several times on different days through a few weeks as I 1. Was concerned about sensitivity in teeth after crowns 2. Needed to make a Cleaning appointment. I always got the Answering Service...and a person who kept me on the phone for very very very long periods of time ...stating that he would give them a message. This happened at least a half a dozen times. I NEVER GOT THROUGH nor did I get a phone call back...even though the message I wanted to get to ******************** was probably not received. I got no response...after leaving various messages...even one of URGENT request.

      For over 1 year, my spouse and I have had multiple billing issues with this provider. They consistently leave items off of insurance claims that they know will be denied despite having received specific instructions to bill everything to our insurance before billing us. They do not answer phone calls, and they do not return voicemails. After over 1 year of fighting with this provider, we filed a complaint with our insurance company and paid the correct balance. I was told on the phone that our balance was zero. My spouse had a dental visit on 1/9/23. We received the first statement for $131 dated 1/15/23. At the time of receiving that statement, no insurance claim had been filed. The balance was sent to collections on 1/27/23, less than 2 weeks after the date of the statement and with no warning. At the time we received the initial letter from the collections agency, our insurance shows a claim of $129 for that date of service that is pending as of 2/4/23. Note that the amount billed to the insurance does not even match the amount in the statement sent to us. I have wasted hours upon hours of my time dealing with this company.

      I had complications after my surgery and was told my **************** who performed the surgery that it was a tooth issue and recommended I see an Endodontist. I payed out of pocket to see 2 different endodontists who told me I had a gum issue and not a tooth issue. When I went back to ****************, she told me it was still a tooth issue and ignored my messages and calls. The management at perfect teeth also ignored my calls and emails until recently when I was able to see a different periodontist who told me i had a gum issue and NOT a tooth issue and that I needed additional surgery to correct the mistakes and neglect from **************** who performed my surgery.

      This business has failed to complete dental work on their patients that has already been paid for. My son has not been seen in over 3 months and needs to have his braces adjusted and fixed. i need to have my braces fixed as the wires and brackets continue to fall apart. they have become negligent on their part of taking care of their patients. you can never speak to anyone and they never seem to follow up with you about anything. if you do manage to get ahold of someone they tell you they will see if they can get you appointment but nothing ever comes of it. it seems like they are avoiding patients so as not to have to help them. you get no explanation as to when you will see anyone. all you are told is that someone from an office will contact you and that their answering service will send an email but the answering service does not even ask for your info so how will anyone know whom to contact. their recent practices are seeming to be very fishy and making patients feel like they are being taken for their money. i am seeking a resolution that gets me and my family the care that we should be receiving or my money back as they are not upholding their end of completing the job. thank you.

      On 12/14/2022 I paid $2,447.28 to Perfect Teeth in ****, ** for 2 partials but they caused me tremendous of pain even after half of dozen appointments for adjustment. They agreed to refund me, but I received a check only $611.82. I tried to contact the office so many times but no response. I found several physical addresses and mailed the refund request to, but one was returned (***********************************************************, their phone ************ was no longer in service,) and no response from the other (found on the refund check: Element Dental Partners ******************************************************************) On January 13, 2023, I sent a certified mail to the office in ****, but it was returned as Unable to forward. I was very frustrated with Perfect Teeths way of conducting business, poor communication, neglect, and unprofessionalism.Attached are the bill that I paid upfront, letters sent to Perfect Teeth's addresses, proof of mails returned.

      9/14/22 ************** at Perfect Teeth (661 S. Bdwy, ******* ** *****) recommended a tooth guard; said it wasn't covered by insurance but would only cost $186.50.10/11/22 tooth guard finished, paid $186.50; 11/15/22 received bill for $611.50 Shortly thereafter I went to the office and inquired--I was told the $186.60 was only half of the charge >if< it was accepted by insurance, the addtnl $425 was charged because insurance rejected it. It should not have been submitted for an insurance claim, so I negotiated with the receptionist. She said she would waive the $425 if I paid the addtnl $186.50 immediately. I felt pressured, but paid it and said I would pursue. I filled out an online form at Perfect Teeth's website asking how to pursue a complaint, but got no response. 12/6/22 I wrote to **************. He called me and said that although he agreed with me completely, he had nothing to do with the billing at Perfect Teeth. He said they had had some issues with billing around that time, but the most he could do would be to refer the matter to a corporate entity overseeing Perfect Teeth. I heard nothing subsequently from them. 1/5/23 I forwarded my complaint to the Compliance Officer of Perfect Teeth (address and name on their website). 1/18/23 The letter was returned to me as "not deliverable as addressed". Not only am I unhappy with **************' hapless response, I am entirely unimpressed by efforts to contact Perfect Teeth's corporate entity.,

      On March 1, 2022, I made a payment of $35 at the Perfect teeth at ********* in ********, **. The payment was for fluoride, I used my credit card to pay, but the person at the front desk said that the payment should show ** in a few days. On March 14, 2022, they called my brother, charging him again for the fluoride, they told him that I owed $35. So, he was charged additionally for the amount that I had payed already, I did no longer owe, but they made an unnecessary charge. I called to let them know about they're mistake. They told me to email them the transaction pictures and I did. I called many times last year and they always said that they would email corporate and get back to me. They wanted my brothers card, but I let them know that it expired, and they didnt offer any solution. I called today to let them know that it's almost been a year and asked if a credit was available since I haven't received a refund, but ***** who I spoke to today said that it wasn't possible, and kept making excuses on why I can't get a refund. I asked for corporates number, but they always tell me that they only have an email. Its very unprofessional that they cant to seem give back $35. Also their customer service is very bad, they cancel patient appointments for no reason as to why and they hardly answer the phone.
