      USA veins clinic has a fraudulent claim I have not received nor requested services from this provider. When calling to obtain clarity how to correct the issue There isnt a clear answer other than if you pay us we are willing to clear your name. How can i pay for services that I have never received nor requested.

The patient was seen in our St. ******** Medical of ******** location by ******************************** 
       
      After thoroughly reviewing, ********************************* was seen for and appointment on 07/26/2022 where a bilateral duplex scan was performed . We billed *********************************, ***** and received the explanation of benefits for the above appointment informing the amount due of $821.39 applied towards coinsurance and deductible per ************************************* benefit plan. We have contacted ***** and confirmed the claim was processed correctly. Therefore, we have concluded the above charges are valid per the financial agreement.

      thank you for the prompt response.

      This Information is inaccurate and untrue.

      I specifically requested verification of coverage prior to any procedure and was told by both your front desk and the medical Provider at the ***********  

      1.this was part of the initial assessment. 

      2. patient has no financial responsibility as initial assessment it is covered by insurance. Just sign this paperwork we will submit it right now and get it done, we take care of everything.
      Patient with your insurance is not financially responsible. 

      3. I found the insistence of the team sketchy , strange and scamming. I proceeded to get up  from the exam table decline services and walked out never went back.

      4. I  have since been bombarded with phone calls , claims and request for follow appointment from your practice. 

      5. Thereafter , upon speaking to front desk staff at the St ******** practice , I was threatened, and told that if I failed to come in for services that I would be billed it was in my best interest just let them finish the job, so we can bill your insurance and we dont have to charge you thats how we do it here

      Clearly *********** is not in the best interest of a patient in any way shape or form. Its a complete insurance fraud business company.

      I had a 12/15/**** clinic visit in ***********, ** at USA Vein Clinics. My insurance 100% Out of Pocket maximum had been met for ****; therefore, I would not be responsible for paying any copays. I explained this to the front desk person, but they forced me to pay the $60 copay or they would not allow me to be seen for my appointment. Once the claim was processed by BCBS, the *** specifically states there is a $0 copay due. Since March 6, 2023, I have called the ****************** multiple times and they continue to give me the run around and promise to call me back, but they NEVER call back! I have left several messages for the Billing Supervisor, *******************, and he also has not returned my calls. I contacted my insurance company and they confirmed that the clinic should reimburse me for the $60 copay amount I paid on 12/15/22 since it was $0. This clinic is keeping funds that should be reimbursed to patients, which is fraudulent. Please assist me with recouping this $60 amount from the clinic.       I called this business to schedule an appointment for a consult. I asked when making the appointment if they are a participating provider for my insurance and they said yes. The day I was seen 7/31/22, they performed an ultrasound and then I consulted with a physician about needing treatment. Shortly after that visit, I received a bill for $1,144.00 for that ONE visit. I called billing and asked what this was that this must be a mistake and they said no, that the provider is out of network and I am responsible for this amount to be paid. I called my insurance company and asked for an advocate. They called me back and advised me to stay away from this company that they had such a difficult time communicating with the people there. My insurance advocate told me that the clinic/facility is a participating provider but the physician that they assigned me to for my appointment was not. My insurance advocate told the USA vein clinics to reissue the claim with just the facility and that they will cover the entire amount due. USA vein clinic has not reissued the claim. To this day, I continue to get calls from them asking about how I am feeling and wanting to know why I have not scheduled an appointment for treatment. I have made more than four different statements with representatives from USA vein clinics about my issue with them. I also continue to get calls from the billing department about this bill. I have tried multiple times to explain to them that they just have to reissue the claim differently to my insurance and my insurance will cover the cost but they said it is my fault this happened and I have to pay the bill. They are rude, disrespectful and frankly very uneducated and unprofessional people. I have one of the best health insurance companies in the country. There is NO reason why I should be in this situation. I would never go to a physician that is not a participating provider. There are plenty of participating providers for my insurance company.

We have confirmed with *********************************** insurance company United Healthcare that our claim processed correctly leaving the amount due as part of *********************************** deductible per the benefit plan. Our records do not indicate any discrepancies when billing our claims as we are a participating provider with the insurance plan.       I am rejecting this response because:

      The business is completely incorrect with their response. The part that is correct is that they are a participating facility however they fail to mention that the doctor is not a participating provider. I asked for a participating provider and they did not give me a physician who was a participating provider. My insurance company asked the **** clinic  for the claim to be submitted differently with the facility and that they would happily pay the entire amount owed. I have had this excellent health insurance for 18 years I do not need to use ANYONE that is not a participating provider. The person who booked my appointment incorrectly connected me with a provider that is not a participating one. That is not my fault. My insurance company agrees as well. They also have tried calling the **** clinic several times to have them resubmit the claim and the **** Clinic has not responded. This company does NOT care about logic what is right for people. This is unfair. Please resubmit the claim with a different code under the facility and you will get your money.

      We are in receipt of the rejected response from the above referenced complaint from your office regarding our patient ********************************

      The member services representative has misled *******************************. I have attached the printout from United Healthcare provider portal eligibility check for date of service 07/31/2022 as well as the electronic claims detail submitted to United Healthcare. The documents clearly show that we are in network with submitting claims the patient's insurance plan under the tax ID and rendering physician.

      ******************************* needs to set up a conference call between United Healthcare member claims representative and myself and request that her insurance reprocesses the claim. I will be available the first week of August to have a scheduled conference call. ******************************* should contact me via email or mobile phone with ************* of the conference call with member services.

      As for the request from the insurance company requesting the provider to fraudulently change the provider billing information, that is unacceptable and should be investigated. We do condone altering claim data in order to satisfy a complaint because the insurance company made an error in processing a claim.

      Thank you for your response. While I am pleased that someone from the *********** is finally looking into this, I am not sure I understand what ********************** is saying. 
      After all this time, is he saying that **** clinic correctly gave me an appointment with a physician who is a participating provider and that my insurance company incorrectly processed the claim?


      I called countless times to billing at **** clinic and was spoken to very unkindly and no one was helpful. They kept telling me that the rendering physician was not a participating provider. **** clinic then continued to call me weekly to try to book more appointments and to ask me how I was doing and if I needed more services. Each time they took my concerned statements regarding the ONE visit I had and still no one could help me figure out this issue. 


      I also spoke with my insurance, I got an insurance advocate and that person had difficulty with speaking to anyone at **** clinic. They hung up on my advocate several times and were not able to figure out the issue. This is the main reason why I chose to go through the Better Business Bureau. And still the Better Business Bureau was unable to reach anyone for quite some time after I submitted this complaint. Better Business Bureau said there was no response from **** clinic. Then some time later, someone from **** clinic responded and wrote incorrect information about my situation. Now, finally ********************** seems to have figured out that there shouldnt be a billing problem at all? If that is correct, then is it possible for ********************** to contact my insurance so they can speak to one another for the claim to be reprocessed and this can be resolved?

      I dont have an actual date because the service was rendered yrs ago. I went for a simple evaluation of my veins at USA Vein Clinics nothing more. I provided my health insurance and was told all i needed to do was pay a co payment which I did. After almost a yr later after my visit they started harassing me stating I owed them hundreds of dollars and i attempting to contact them but to no avail the harrassment continued. Now suddenly this yr I see this company has placed a negative **** on my credit report . These people are scammers they get people to sign up for their services stating they take insurance they are misleading people with deceptive practices.

      I received treatment for veins in both legs last year, 2022, starting in May and ending in August. I had a co-pay of $90 that I paid at time of service each time I went. In November I received an email stating that there was a discrepancy with my insurance and that I had a balance of $90 left on my account and that I would be charged automatically with the card they had on file if I didn't respond within a certain amount of time. I didn't get to read the email in time and my account was charged. I noticed because they actually charged my account three separate times for $90. This caused my account to go to overdraft. I called the local number and asked about the charges The representative told me the charge was for an unpaid co-pay. I thought I had paid each time but I was told one of the visits when I was charged it was covering the first visit's co-pay so that appointment had been left unpaid. It was determined that only one $90 charge was legitimate and the other two were made mistakenly. The rep told me $180 would be refunded to my account but it could take **** business days. The charges stayed as pending on my account (even thought the money had already been withdrawn) by the end of the week all three charges had cleared. I did not receive a refund in **** days so I called again. The local number was terrible to connect with, hanging up on me and being put on hold for long periods. I finally started calling the corporate office and spoke to a rep in Jan that told me I was owed a refund of $180 and that the formal complaint would be made and finance would contact me. I was neither refunded nor contacted. I called again in Feb and spoke to a new rep that told me the three payments were found voided on my account but not on the transaction side causing the charges. I spoke to the rep twice that month, and even provided a bank statement. Now I am being told refunds are a different department. Still no refund or call. They have records of each call I make.

      I went for a consultation visit on 10/12/22 and was told my insurance was in network, however the clinic thought otherwise and billed my credit card $343.00 (11/23/22) to the *********** Medical Bronx ** and a $25 copay on (10/14/22) to the same on Oct. 14, 2022.       The patient was seen in our Pelham Parkway Medical of ******** location by Dr. ************************************************** style="font-size: 0.875rem;">Ms. *********************** has a signed financial agreement informing her that by providing credit card information, which we securely store electronically, she grants permission to automatically charge the credit card on file for applicable charges including co-pays, coinsurances and deductibles that are due.

      After thoroughly reviewing, Ms. **********************;was charged $343.20 pertaining to an appointment attended on 10/12/2022. We submitted a claim to Ms. *************************** insurance company United Healthcare and received a response stating a balance of $1,144.00 was left as part of a deductible per the benefit plan. Our records indicate Ms. *********************** paid $25.00 on 10/12/2023 which was applied towards the $1,144.00 amount due leaving $1,119.00. We billed Ms. *********************** the information for the amount due with no response which caused the automatic charge of $343.20. We then received a notice by United Healthcare indicating the claim was sent for reprocessing and the patient responsibility was adjusted to $50.00. Our records indicate the insurance payment was received successfully.

      I was a patient of Vein Clinics of ***** and had been for about ************************ my current state, I had not been for treatmenet since before ***** but had been doing well. Suddenly in 2/23 I was having problems and came to discover that Vein Clinics of ***** were purchased back in October of 2022 by USA Vein Clinics. I had absolutely no notification of any kind and was not given the opportunity to collect my very vast set of medical records. Someone must be responsible to the patients so that they can continue to get treatment elsewhere and the new doctors can have some indication of their health history and the details. If *** is not responsible to communicate to patients, is USA Vein Clinics responsible? I had set up an appointment with USA Vein Clinics and had an ultrasound ($300) which I now regret wasting my money. I had to drive an hour since they closed the *** office in my town then had a three hour experience in the office an hour away where I felt l had been taken advantage of and where not one person in there had a clue what they were doing. The office was almost a ghost town with three employees who were winging it and all came from other states to fill in and see patients. It was clear by the second hour there that I had made a mistake. They claimed they, too, were having problems getting medical records and wanted me to come for treatment the following week. I decided after leaving there that I did not feel comfortable with any of it. They have battered me with texts, phone calls and emails saying just contact us or respond to this...I did that in every way to cancel my appointment and they kept calling me. A different person each time, just a call center. They even decided they would reschedule me without my consent and kept calling about that appointment too. I finally found a live person to tell them about my bad experience and I wanted no more phone calls or messages. They hardly cared that I had a bad experience and did not ask about it. I would like someone to make them send me my medical records!

      Back in 2018 I did vein treatments in my both legs by the time I have horizon *********** my copay was $88.Now after 5years they sent me a bill for 14k after discussing with their manager ******************* several times he said we made a mistake your balance is 3k .       USA Veins has not supplied me with an itemized bill, in spite of me asking them to do so repeatedly. Specifically, I asked for an itemized bill for two charges:- 1/18/2023 for $305 - 2/1/2023 for $305 On March 3, 2023, I spoke to the supervisor, *****************************, and asked for itemized receipts.On March 13, I spoke to ****** He sent me over to the ******************** to request an itemized receipt. I asked why it had to go through the insurance department when I wasn't going through insurance but, rather, paying myself - he didn't know why, but said the request had to go through the insurance department.March 22, I called and spoke to ******* but the call was dropped. Called back and spoke to ***** and asked to talk to the billing department. I was transferred and spoke to ****** I asked him for itemized bill. He asked for my date of birth so he can access my account, and said he will send my bill for the date range of January 18 - February 2. I asked how I can be sure I'll get it. He said they always send it, and that they send it through US Mail. I said no they don't. I explained that this is my third request for a bill, which is a follow up to another issue about which I had called several times. I also told him that if I do not receive my bill in a week, I will report them, which is what I'm doing now.

      I was a patient of Vein Clinics of *******. I was being treated at my local ****** in *********, **. My last appointment was supposed to be on 11/8/22 but I got a message that Vein Clinics of ******* was taken over by USA Vein clinics and they would have to reschedule my appointment because the local ****** near me was being closed down. Their closest ****** to me was in ****, ** which is about 3 hours away. At that point I asked for my medical records to be mailed to me because I couldn't travel that far and I would have to continue with a different doctor's ****** nearby. It is now the end of March and I still haven't received my medical records which I know I'm entitled to. I've tried emailing the company, calling and texting. I can't complete my treatment without those records.This company is very unprofessional as I've been reading in many complaints filed with the BBB.Please advise....thank you
