      I had a blow to my jaw in a fall which resulted in 2 crowns coming off. I have Delt Dental insurance from my Medicare Advantage and Florida Dental Centers in a approved provider. I call Florida dental centers and was given a $74.00 per tooth estimate by *****. I called Delta Dental to see if the services were covered they stated they were not. I went into Florida Dental centers 9/11/2023 to have the teeth recemented. I was given a printed financial statement of 2 crowns re-seeted and one x ray. I was also given an estimate of an UPCOMING appointment estimate. The crown work and 1 bite wing x-ray totaled $200.00 on the estimate. The upcoming future work was an additional over $200.00 When I went to check out and pay the $200.00 ***** stated my portion was entirely covered. The estimate had all 0 next to the work both present and future. I then received a denial of coverage notice in the mail from Baycare plus stating the work was not covered by Delta Dental. Florida Dental centers had placed the future work on the current work charge. I was billed for 2 exams and 7 x rays!! I called Florida Dental centers and spoke with very unprofessional Mandy who refused to put ***** on the phone to discuss the denial, then she stated I had signed the financial responsibility form so I was obligated to pay the $400.00 charge my portion was first she stated $144.00 then she changed this $158.00. Florida Dental center fraudulently billed my Medicare Advantage provider Delta Dental for TWO examinations and 7 x rays including all the FUTURE work in the bill. I then called Florida Dental Centers corporate and was told a regional person would contact me after examining the date of service and all the issues and concerns with the Ulmerton branch and Mandy the center manager. 14 days later with no call I am now being told the person I spoke with at Florida Dental Centers resigned and no one has addressed my issues and overpayment. I am owed money for this date of service. The Florida dental center on Ulmerton Road has up coded and overbilled and falsely explained procedures. In addition the corporate office has not looked into the 9/11/2023 date of service its charges and services as they stated they would. This will also affect the amount of future work I am entitled to a different dental provider. The codes placed for services NOT rendered prevent me from actually obtaining those services they now look like duplications. This date of service needs to be examined the bill pulled from Delta Dental the unrendered services removed and the money refunded This needs to be resovled I am owed money for services not rendered not rendered and I have filled a grievance with my Medicare agains Florida Dental Centers.

Her and I have agreed that she will switch to the Pinellas Park location and continue her serves with Florida Dental Centers, just not the Largo office. As far as the charges on the patients account, she has paid them. She was a little confused as to what was charged out and we have discussed that and she understands now! I have spoke with my regional manager and the Largo Office Manager is being handled accordingly. But Ms. ***** and I have established a good relationship and have agreed we are the better fit for her!

      I came in for my appointment scheduled for 9/18/2023 at 3:00pm. The receptionist asked me if my medical history changed. I said no, then she gave me a new packet of paperwork to complete. I started to complete the paperwork, but did not finish prior to being called back for my checkup. Before the checkup I asked if I could brush my teeth and I was dismissed by the examiner. After the **rays were taken, I asked again if I could brush my teeth prior to the exam. She provided me with a toothbrush, brushed my teeth, and I started to complete the remaining paperwork. Prior to a doctor coming in, a manager came in and said that I needed to complete the paperwork. The paperwork was almost complete, but had a few questions and objections to signing financial statements. I asked about a few items on the forms related to paying in full prior to service, and payment based on notice of service. She (manager) basically just wanted me to sign and not ask questions. Then she made the statement, maybe you shouldnt be a patient here. I was extremely offended. I have not and did not do anything but ask questions related to the paperwork I was provided. I asked her (manager) Are you asking me not to be a patient here? Then she tried to backtrack on what she just told me. I was trying to clarify why she would say something like that to a patient, but then she was just rude. I walked to the front desk and asked for a manager. The front desk representative said that she (the person that staid I should not be a patient) was the manager. I asked the front desk worker to speak to her (meaning that manager) boss or to the corporate office. She said she wouldnt and proceeded to say that we will just send you a termination letter. I asked about the payment for services for the exam that was not completed, and she said there will be no charge. I am having a hard time believing them and want assurances in writing that no charges will go to my insurance carrier or me for an incomplete exam. I would also like to speak to a corporate official that is in charge of the manager who spoke with me.

      I canceled appointments with Dental ************** due the company quoting a different price after signing an agreement. It was explained to them our income a as very limited. I paid $450 in advance to complete the work. Scheduling time came the price increased. I asked for the money back in which I received. September 11, I received another check for $450. I called and spoke to representative and told him I received a check already. He said he see where a check was sent, ot was an oversight on their part and to enjoy the money. I asked him was he sure, he said yes and enjoy my day. I deposited the check into my account. Yesterday I checked my account and it is currently overdrawn $450 I've been trying to reach them by phone, and no one is returning my call. I would like someone to call me from the company.

      I received a bill for $375 from ******** Dental of **********. I will not be paying the bill. I demand that this bill be waived in full, and that my account be set to $0. You told me that my treatment would only cost $250 which I paid. I will not pay any more for your mistake. According to Humana, "After review of submitted periodontal charting and the radiographs, it is determined that there is no attachment loss that allows for the planing of roots." You billed me for dental work that was not needed or necessary. Even Humana has said that the planing of roots was not needed. I paid you $250 already. I am not paying anything more for dental work that was not necessary. It's your problem that Humana did not pay you, not mine. I expect a $0 balance, and an apology from the dentist in writing.

      According to Humana, "it is determined that there is no attachment loss that allows for the planing of roots. The provided documentation demonstrates critical bone interproximally with bone height within 2 mm." ******** Dental of ********** needs to waive my bill in full. ******** Dental of ********** told me that my treatment would only cost $250, which I paid. Your company then billed me for dental work that was not needed or necessary. Even Humana has said that the planing of roots was not needed. I paid you $250 already. I am not paying anything more for dental work that was not necessary. It's your problem that Humana did not pay you, not mine. I expect a $0 balance on my account, and an apology from the dentist in writing.

      The *************** Branch not only billed my insurance incorrectly, they doubled my bill (an obvious and easily fixable clerical error) and are refusing to fix either of those problems. I only received $500 worth of services from them as stated in the invoice and they continue to send me a bill for $1,000. When I call the office they refuse to remedy this issue and refuse to direct me to anyone who can. I have been patiently working with them and though they have said multiple times that their billing department will fix the issue, there are no changes on the account and I continue to receive bills for $1,000 and threats to take this to collections DESPITE the fact that there is a known issue that we are working through. This is unethical and almost seemingly intentional.

      After i signed my contract for my children's orthodontics care, i was informed i would begin to be billed automatically to the card i provided in August 2022. Apparently this was not the case and i was not informed about this until October 2022 when a new manager started and new office staff started. I was informed by the manager that the prvious manager and staff team had a chaotic mess and multiple accounts were incorrect. *** said for now pay your monthly in person or over the phone until we correct the account. Months went on with continued issues and being double billed and at one point even the incorrect insurance information was input into my kids accounts. It has gotten so bad that even my dental insurance company is now currently withholding payment as of August 2023 until the Dental office answer a series of questions of when service originally started and what is care plan. On Saturday August 19th i received a final notice bills for 2 of 3 of my children. So today on Monday august 21ST When i called the dental office today to get to the bottom of the problem once again, i was treated rudely and hung up on. I have been beyond patient and on a monthly basis HAVE BEEN TRYING TO GET MY KIDS ACCOUNTS AND BILLING RECTIFIED. This office continues to double bill me, inputting incorrect insurance company information that i never even provided to them, and then they don't submit the required paperwork to my ins. company when it is requested.

      On 6/29/2023 I was pressured into a contract with the orthodontist because I was told that my son needed braces and insurance didnt cover it and tho I had a savings plan the billable amount to insurance is more than the self pay amount and the only way I would be able to obtain the savings I would need in order to afford this would be to sign up that day same day consult savings of $500. The total for my sons procedure would be for $5,925.00. Since I agreed to this my appointments are getting canceled and my sons treatment plan is being pushed back. My son has only received his first row of braces though the full amount has been paid, every time I call the office I get voicemail theres no communication and I was told if my son misses any appointments his treatment plan will be extended and Ill have to pay more out of pocket. Cant get anyone on the line to assist me and Im out $5.5k I would like a refund credited back my credit card so I can seek my sons treatment elsewhere.

      I was contacted by **** with Main Street Children's Dentistry And Orthodontics Of Ft. Myers today September 8, 2023 @ 12:18pm he apologized for the mistake and inconvenience and cancellation of my appointments. He noted that he received a second letter from BBB, he noted that at this time theres a lot happening within the practice causing these issues with scheduling. He asked what could he do to resolve the matter. I noted I would like my money back, he noted at this time he would have to wait to speak to the provider. No refund was provided.

      On 2/21/2023 my general dentist identified a *************, #**, that needed to be removed. They suggested I contact a dental shop that would remove and do an implant.On2/2/2023, the Made Ya Smile dentistry at **************************************************************************************************************** examined and agreed and scheduled an extraction on 3/13/2023. On that date a Dr. **** *******, at the same location, pulled tooth #**. I complained of the same type of pain after the incorrect tooth was pulled. I was X-rayed on 3/14/23 and it was verified the wrong tooth was pulled.Dr. ***** of Made Ya Smile Dental pulled the correct tooth, #**, on 3/21/23. I was told 4 months was needed for the bone to heel. I had a scan performed on or about July **th. I was told no one at Made Ya Smile density wants to do the work and they claim they are filing a dental malpractice claim alim in Dr. ******* before this can be resolved. Repeated calls post July **th have gone unanswered.

      I was seen by ************ ( part of Dental Care Alliance) on 11/15/2022 for a lesion on my tongue. At tht time he indicated that it needed to be biopsied, the office staff gave very specifically told me that insurance would NOT cover the cost of this procedure and they said the cost was estimated at $781.00. On 12/13/2022 I was went to Dr. ***** office for the biopsy of my tongue, prior to even having the procedure, the office presented me with a bill and told me the total cost to me would be $624.80. I paid the office that day $455.20 on my credit card and previously I paid a bill from them for $169.60 which was then paid by my insurance so instead of funds sent back to me I had a credit of $169.60 which was applied to the bill for a total that I paid for that procedure of $624.80. As the office told me insurance would NOT cover any part of procedure I thought I was totally done with the situation. At my request I asked the medical portion of my insurance to see if they would cover cost as this was a medical biopsy, not a dental procedure. The insurance company ultimately paid a portion of this claim. They paid to the dentist $109.95. When the dental office first sent me a bill of $46.25 I called, then emailed 3 times then sent message through their office system it was not until 6/24/2023 that I finallyl received a response from them after I had to show up at their office. At that time they said that the bill was just an estimate and the insurance allows them to bill up to $781 for the procedure. This felt like fraudulent practice. If you tell me an estimate is a certain amount and day you do the procedure you say amount I owe is something other than estimate I believe the change is due to scope of work is less than expected. Certainly not well we didn't know if we could bill you more than you paid but since we can now you owe us more. also they accepted payment from *************** as per their contract they should be bound by terms of contract with Humana.

      On March 6, 2023, I went to Dental one to have my extra pair of denture to be repaired. prior to this I had called to make sure that i could drop my upper denture off. I asked beforehand would i have to be seen. I was told that because I was dropping my dentures off I would not have to be seen. Upon my arrival I was taken in back still with my dentures in my hand. I was then told that i had to have a **ray done before i could speak to the Doctor. I tried to explain to the assistant that i was just dropping off my dentures to have the loose teeth repaired. She stated that she would have to speak to the Doctor, a few minutes later she came back and said that I had to have the **ray done before i could have my dentures repaired. So that was done , then I had to have a Oral Evaluation done even though, this have already been done by this dentist before.I received a bill from Dental one $181.50 after my insurance paid towards the Panoramic Radiographic Image and denied the exam. Because as i stated before I have had a Oral Evaluation by the dentist before.I do not understand why it was necessary to have a ORAL EVALUATION.Why was it necessary to have a Panoramic Radiographic Image done When I was only dropping my Dentures off to have the tooth repaired I tried reaching out to Dental one about these unneccessary procedures. A few days later I received a letter banning me from their facility

Despite our commitment to resolving consumer complaints, as a management company, *** must also adhere to protecting the privacy, confidentiality, and security of health information. As a "covered entity," *** is governed by privacy regulations including the **************** Portability and Accountability Act ("HIPAA").Given the requirements of HIPAA and other privacy laws and regulations, *** cannot fully respond to the complaint through the Better Business Bureau forum,as in order to do so, *** would need to provide the Better Business Bureau with detailed information about private health information and the facts and circumstances of a particular case. In cases where removable prosthetics (dentures) are being made or repaired, an exam with a panoramic x-ray is necessary to provide a complete view of the patients jaw,arches, and tissue to ensure proper alignment, retention, and overall fit.

      I am rejecting this response because:
      As I stated before, the xray was not necessary. 

      The dentures was not being fitted , they were being sent out to have a tooth re- attached to the denture.

      There was no resized done at all. When I picked up my denture after the repair I did not see a dentist at all

      My denture was given to me in my hand not placed in my mouth and I was told to have a nice day

      Yet you are saying all these exams was necessary just to repair a loose tooth on my denture
